翻瓣与不翻瓣种植对前牙美学区修复效果及周围组织影响的临床研究(1)
[摘要]目的：探究前牙美學区翻瓣种植、不翻瓣种植对种植体周围组织的影响及临床效果。方法：选取2013年1月-2016年1月笔者医院76例行上颌前牙区单颗种植的患者为研究对象。按随机数表法分为两组，研究组(n=38)行不翻瓣种植，对照组(n=38)行翻瓣种植。随访12个月，观察两组骨吸收、种植体周围牙周组织情况及红色美学评分(PES)、术后不良症状发生率。结果：修复后6个月、12个月研究组唇侧骨吸收、舌侧骨吸收低于对照组(P<0.05)。修复即刻研究组改良龈沟出血指数低于对照组(P<0.05)；修复后6个月、12个月研究组龈乳头指数高于对照组(P<0.05)。修复即刻两组PES评分比较差异无统计学意义(P>0.05)；修复后6个月、12个月研究组PES评分(10.08±1.24)分、(12.17±1.58)分，高于对照组(8.65±1.01)分、(9.72±1.13)分，差异有统计学意义(P<0.05)。研究组术后出血(2.63%)、红肿(5.26%)、疼痛(5.26%)率低于对照组(15.79%、21.05%、21.05%)，差异有统计学意义(χ2=3.934、4.145、4.145，P<0.05)。结论：前牙区不翻瓣种植的美学效果更理想，患者术后不良症状少，安全性高。

Abstract： Objective To investigate the influence and clinical effect of anterior aesthetic region valvuloplasty and non-valvular implantation on peri-implant tissue. Methods From January 2013 to January 2016，76 patients with single implant in maxillary anterior teeth were selected.The patients were divided into two groups according to the random number table method. The study group (n=38) received no valvular implantation and the control group (n=38) was treated with valvuloplasty. Follow up for 12 months， Bone resorption， Periodontal tissue around implants and red aesthetic score were observed in the two groups. The incidence of postoperative adverse symptoms was observed. Results After 6 months and 12 months repair， the labial bone resorption and lingual bone resorption in the study group were lower than those in the control group (P<0.05).The modified gingival sulcus bleeding index in the study group was lower than that in the control group (P<0.05)， and the gingival papilla index in the study group was higher than that in the control group at 6 months and 12months after restoration (P<0.05).There was no significant difference in PES score between the two groups immediately after repair (P>0.05)， but at 6 months and 12months after repair， the PES score of the study group was( 10.08 ±1.24)score，( 12.17 ±1.58)score， which was significantly higher than that of the control group (8.65±1.01)score and (9.72±1.13)score. The difference is statistically significant(P<0.05).In the study group， the rate of bleeding was 2.63%， redness and swelling were 5.26% and pain was 5.26% . The rate was lower than that in the control group (15.79%， 21.05%， 21.05%) . The difference is statistically significant(χ2=3.934，4.145，4.145， P<0.05).
