面部脂肪室的解剖及临床应用研究进展(1)
[摘要]通过查阅与面部脂肪室相关的文献,对面部脂肪室相关解剖研究、脂肪室在面部老化过程中的改变及其临床应用进行综述,分析总结其研究进展及临床应用。既往面部脂肪被认为是以整体存在,并且在面部衰老过程中发生着同样的变化。但近年的研究表明,面部脂肪是以区室化形式被分割成多个独立的脂肪室,并且在面部老化过程中,浅层脂肪室的体积随年龄增大而变大,而深层脂肪室的体积随年龄增大而变小,并且在临床中可通过对不同层面的不同脂肪室进行填充,从而精准地实现面部年轻化或改善面部轮廓。面部脂肪室的出现为研究面部老化机制提供了新的探索思路,也为实施精准的面部年轻化提供了理论基础。

Progress in Anatomy and Clinical Application of Facial Fat Compartments

Abstract: To review the progress of anatomy related to facial fat compartments, the changes of fat compartments during facial aging and its clinical applications. The literature related to facial fat compartments was reviewed, and its research progress and clinical application were analyzed and summarized. In the past, it was thought that facial fat was present in a manner, and the same changes took place during the aging of the face. However,recent studies have shown that facial fat is divided into a plurality of independent fat compartment in the form of compartmentalization, and in the facial aging process, the volume of the superficial fat compartments increases with age, and the volume of the deep fat compartments decreases with age. In addition, the filling of different fat compartments at different levels can precisely achieve facial rejuvenation or improve facial contours. The appearance of facial fat compartments provides a new exploration idea for the study of facial aging mechanism, which provides a theoretical basis for the implementation of accurate facial rejuvenation.

實现面部年轻化是整形美容外科的永恒话题。面部衰老主要体现在皮肤、皮下软组织以及面部骨骼的变化。随着年龄的增长,面部逐渐出现皮肤变薄、胶原丧失、皮肤弹性变差、脂肪容积及位置改变、肌肉萎缩及骨骼吸收等,这些改变导致面部逐渐老化,并出现各种皱纹、皱褶[1]。而脂肪作为面部软组织的重要组成部分,它在面部老化过程中扮演着重要角色,面部脂肪的改变直接影响着面部轮廓的改变。
随着脂肪移植的关注度提高,人们开始探索面部脂肪的存在形式在面部衰老过程中发生的变化,以及如何通过面部脂肪来实现面部年轻化。2007年,Rohrich[2]首次证实了面部脂肪室的存在,从而为后续面部脂肪室的研究提供了理论基础。本文主要对面部脂肪室的解剖、面部脂肪室在面部老化过程中的变化情况以及如何利用面部脂肪室进行面部年轻化等相关研究综述如下。
1 面部脂肪室的解剖
2007年,Rohrich[2]等利用亚甲蓝对尸头面部特定区域的脂肪组织进行染色,然后通过解剖观察,从而首次证实了面部脂肪是以“脂肪室”的形式存在的。Rohrich[3]等对面部脂肪室作进一步研究,发现皮下脂肪室彼此间被隔膜分隔,该隔膜起自浅筋膜的致密纤维结缔组织,并向上延伸至皮肤真皮层,从而形成了面部的“支持系统”,对脂肪室起到支撑、悬吊的作用。Schaverien[4]等通过尸头动脉灌注染色及三维CT扫描的方法,发现包绕脂肪室的隔膜有相应的动脉或静脉在其中穿行,这些穿行于隔膜中的血管发出相应的分支分布至皮肤,从而形成了交错伴行的脉管系统,对面部血供起着稳定性和保护性的作用。, http://www.100md.com(朱伟东 王晋煌 柳大烈)
