    [摘要]目的：探讨自体脂肪精细移植在面部年轻化及轮廓重塑中的临床应用效果。方法：选择2019年10月-2021年12月笔者医院收治的251例需面部自体脂肪填充的就醫者，术中从脂肪供区低负压抽吸脂肪(首选大腿内侧和下腹部脂肪供区)，用纱布吸附过滤法充分去水去油提纯浓缩颗粒脂肪，去除纤维筋膜，根据不同部位使用不同孔径注脂针结合面部解剖及老化特点选择层次精细化注射，术后随访半年至1年，对手术后效果进行总结。结果：所有就医者术后肿胀轻微，肿胀恢复期7～10 d，不超过14 d，无感染、血肿、栓塞、脂肪液化、皮下硬结、术区凹凸不平等并发症发生，手术总有效率为97.61%，就医者满意率达92.03%。结论：面部脂肪精细化移植能有效改善面部老化症状，重塑面部轮廓，就医者满意率高，安全性好，值得在临床推广应用。
