【摘要】目的:探讨食物过敏反应机制。方法:对64例肠易激综合征患者和正常对照组30例,进行食物特异性IgG和IgE检测,血清总IgG和IgG亚型测定,通过饮食回避试验和激发试验确立食物过敏者,探讨食物过敏反应机制。结果:①14例食物过敏者中有1例食物变应原特异性IgE检测为阳性且食物变应原特异性IgG检测和IgE检测一致,其余13例均仅为食物变应原特异性IgG检测结果阳性。②14例食物过敏者血清总IgG、IgG1、IgG2和IgG4平均浓度和正常对照组比较有显著性差异,IgG3和正常对照组比较无显著性差异。③14例食物过敏者血清总IgG、IgG1、IgG3平均浓度和特异性IgG检测阴性病人比较有显著性差异, IgG2和IgG4与特异性IgG检测阴性病人组比较无显著性差异。结论 慢性食物过敏者中存在IgG介导的变态反应和IgG亚型的变化,由IgG介导的变态反应占主要地位,而由IgE介导的变态反应占次要地位。

【Abstract】Objective:To observe the mechanism of food allergy. Methods:The food-specific IgG and IgE antibodies to a panel of food antigens in serum, total serum IgG and subtype of IgG were detected in sixty four cases with IBS and thirty normal controls. The avoiding diet test and the stimulating diet test identify patients with foodhypersensitivity in order to explore the mechanism of food allergy. Results: ①One of case 14 patients of food allergy was positive in food-specific IgE test.At the same time,it had same result in Food-specific IgG and IgE. ②The average concentration of total serum IgG, IgG1,IgG2and IgG4of patients with food allergy had significant difference compared with that in normal controls, the average concentration of IgG3 of patients with food hypersensitivity had no significant difference compared with that in normal controls. ③The average concentration of total serum IgG, IgG1,IgG3of patients with food allergy had significant difference compared with that in negative group with IBS of food-specific IgG test, IgG2and IgG4 of patients with food allergy had no significant difference compared with that in negative group with IBS of food-specific IgG test.Conclusion:Patients with chronic food allergy has IgG-mediated allergy and subtype ofIgG changing. Food allergy played an important factor in the pathogenesis of IBS.To patients with chronic food allergy, IgG-mediated food allergy dominated food allergy, IgE-mediated allergy is secondary in food allergy.
