局部氧疗在高原地区治疗糖尿病足坏疽疗效观察(1)

【摘要】目的：探讨局部氧疗辅助治疗糖尿病足坏疽的疗效。方法：对41例糖尿病足坏疽患者随机分为对照组和治疗组，对照组20例患者32个创面，给予基础治疗和外治法治疗；治疗组21例患者30个创面，在基础治疗和外治法治疗的基础上治疗组每天局部氧疗1次，每次持续60min，每周治疗6天，4周一疗程。比较两组创面愈合所需的时间,创面愈合总有效率，并作统计学分析。结果：对照组创面创面愈合总有效率为65.63%，愈合时间为(24.89±4.23)天，治疗组创面创面愈合总有效率为93.33%，愈合时间为(18.77±4.63)天。对比项目在两组之间差异有统计学意义(P<0.01)。结论：局部氧疗可以加速糖尿病足坏疽创面的愈合,并且简单易行，无全身性的副作用。

【Abstract】Objective:Adjuvant treatment of local oxygen therapy of diabetic foot gangrene patients. Methods:41 cases of diabetic foot gangrene patients were randomly divided into control group and treatment group and control group of 20 examples with 32 wounds received conventional therapy; treatment group of 21 examples with 30 wounds on the basis of routine treatment of local oxygen therapy in treatment group 1 day times, each lasting 60min, 6 days per week treatment, 4 treatments on Monday. Two groups were compared the time required for wound healing, the percentage of wound healing, treatment efficiency, and for statistical analysis. . Results:Control group, wound healing time was (20.1±3.1) days, the treatment group (13.0±2.7) days; Wound healing in the control group was (71.8±12.1)%, the treatment group (96.1±5.5)%; The control group was 65.00% effective wound treatment, the treatment group was 100.00%. All items in comparison between the two groups were statistically significant differences (P<0.05). Conclusion:Local oxygen therapy can accelerate wound healing in diabetic foot gangrene, and simple, no systemic side effects.

糖尿病足坏疽是Ⅱ型糖尿病的严重并发症，坏疽期可出现干性溃疡和湿性溃疡，组织缺血坏死，合并感染，甚至需要截肢。我省地处高原地区，海拔2261米，由于高寒缺氧致创面局部氧供不足，组织修复缓慢，愈合时间延长。现对我院2007年1月～2010年10月住院的41例糖尿病足坏疽患者患者，在常规治疗的基础上加用局部氧疗，收到良好疗效，现报道如下。
1 资料与方法
1.1 一般资料:41例住院病人，符合1997年ADA诊断标准〖1〗。按随机分组设计原则分为治疗组例21例，对照组20例。治疗组中男性16例，女性5例；年龄42～83岁，平均(59.9±12.7)岁；糖尿病病程3～24.5年，平均(8.1±1.9)年；糖尿病足坏疽病程9～325 d，平均(69.4±15.7)d；来院时空腹血糖水平9.9～17.9 mmol/L ......
