本研究发现ICAM-1在乳腺IDC癌巢中的表达阳性率与淋巴结转移呈负相关,而与TNM分期及组织学分级无关。Ogawa等发现ICAM-1的表达与肿瘤大小、淋巴结转移、肿瘤浸润、核多型现象和核分级呈负相关[7]。Madhavan M.等也观察到在淋巴结阳性乳腺癌组织中其ICAM的表达低于淋巴结没有转移者[8]。其机理可能是乳腺癌细胞的ICAM-1表达缺失或下调,使癌细胞之间的黏附性降低,使得癌细胞容易从癌原发灶中游离,当游离的癌细胞通过淋巴管被引流至淋巴结时,由于ICAM-1的表达缺失或下调,它们逃脱了宿主的免疫监视系统,不被淋巴细胞识别和溶解,从而发生淋巴结转移。有研究表明有抗肿瘤作用的TNF-alPha,托瑞米芬,ATRA(全反式维甲酸),IFN-γ等均可上调乳腺癌细胞株的ICAM-I的表达[9-12],通过ICAM-1的上调可显著地抑制了乳腺癌细胞株的生长[9]。这些实验研究也支持上述观点,上调ICAM-1的表达可能减少淋巴结转移,从而改善预后。对ICAM-1作用机制的深入研究有可能使其成为乳腺IDC患者个体化治疗的新靶点。
