摘 要：目的：观察清热利湿活血法(痛风消汤)对急性痛风性关节炎患者血清白细胞介素6(interleukin-6，IL-6)和肿瘤坏死因子-α(tumor necrosis factor-α，TNF-α)的影响，并探讨其治疗机理。方法：将56例急性痛风性关节炎患者随机分为治疗组和对照组各28例，两组均用西药常规治疗，治疗组加服痛风消汤，进行7天临床观察，比较两组治疗前后临床症状和血清IL-6、TNF-α、ESR、CRP、BUA的水平以及关节情况的变化。结果：治疗组和对照组临床治疗有效率分别为85.71%%和67.86%，两者比较差异有统计学意义(P<0.05),两组治疗后ESR、CRP、IL-6及TNF-α与本组治疗前比较差异有统计学意义(P<0.05，P<0.01)，治疗组治疗后BUA、ESR、CRP、IL-6及TNF-α及关节疼痛缓解时间及关节红肿消失时间均低于对照组治疗后(P<0.05)。结论：清热利湿活血法能明显改善痛风性关节炎患者临床症状，其机制可能是通过抑制炎症反应，降低炎症因子的水平以及促进血尿酸的排泄。

on Levels of Interleukin-6 and TNF-α in Acute Gouty Arthrits

Abstract:Objective：To observe the effects of heat-clearing-damp-removing and blood-circulation-promoting method(Tongfengxiao decoction)on levels of Interleukin-6 and TNF-α in acute gouty arthrits as well as its therapeutic mechanism.Methods：58 cases of acute gouty arthrits were randomly divided into two control groups ,which were treated by conventional western medicine, and treatment group(n=28), which was treated by Tongfengxiao decoction combined with conventional western medicine. Clinical symptoms、levels of IL-6、TNF-α、ESR、CRP、BUA and the conditions of arthritis were measured seven days after treatment.Results：Clinical effective rates of two groups were respectively 85.71% and 67.86%, there was statistical significance(P<0.05).There were obvious differences before and after treatment on levels of ESR、CRP、IL-6 and TNF-α in two groups(P<0.05，P<0.01).The levels of BUA、ESR、CRP、IL-6、TNF-α and joint pain relief time and joint swelling vanishing time in treatment group were decreased in comparision with those in control group after treatment(P<0.05).Conclusion：Heat-clearing-damp-removing and blood-circulation-promoting method can obviously improve the clinical symptoms of gouty arthritis patients，its mechanism may be through inhibition of the inflammatory response, reducing inflammatory factor levels and promoting blood uric acid excretion ......
