XE-2100血液分析仪测定NRBC是利用荧光染色对细胞核进行染色,测定其侧向荧光强度(反映细胞核的大小)和前向散射光强度(反映细胞体积的大小),将其与白细胞、红细胞以及血小板区别开,通过流式的技术对其进行计数。本组实验中XE-2100血液分析仪在没有开通NRBC通道时对NRBC报警的灵敏度为100%,没有1份被漏检,10份假阳性标本中8份有晚幼粒细胞,2份有晚幼粒细胞和异常淋巴细胞同时存在,它可能是将这两种异常细胞误认为NRBC,和作者以前的研究结果[2]一致。在开通NRBC通道后,这10份标本的NRBC定量计数都为0。
利用母体的静脉血测定NRBC数可对围产期新生儿窒息进行预报;测定新生儿血液中的NRBC可用于评估新生儿宫内发育迟缓并发症的病情;测定ICU患者血中的NRBC数,可对患者病情及预后进行评估[3-8]。
NRBC在较多血液分析仪中被作为白细胞计数,引起白细胞的假性增高,常常需要通过人工显微镜的方法来校正,但这种方法较为繁琐,灵敏度、特异度、重复性都较差。现在尽管可以采用流式细胞仪来测定NRBC[9],但这种方法测定的时间较长,所用的试剂较多,试剂成本较昂贵,在临床应用中普及较难。XE-2100血液分析仪测定NRBC的重复性、线性范围、携带污染率以及灵敏度都较好,无疑是一种较为经济、简洁、准确的方法,较适宜在临床推广使用。
