3.7 贫血和心肾贫血综合征 2000年以色列Silverberg等[21]正式定义心肾贫血综合征为心血管疾病和慢性肾脏疾病进展过程中都会伴发贫血,且三者相互影响,机体对体内外环境变化作出的反应，三者之间存在相互加重的恶性循环[22]。CHF的低排血量及神经体液异常引起肾血管收缩，导致肾脏缺血，肾功能不全或恶化，促红细胞生成素生成减少，抑制骨髓活性，引起贫血。此外，慢性心衰中的一些细胞因子如肿瘤坏死因子α和白细胞介素6也可直接抑制骨髓及干扰促红细胞生成效应，加重贫血。贫血时由于血管扩张, 心率加快, 反射性心搏量增多, RAAS系统的持续激活,致心脏负荷加重,左心室肥厚,心肌细胞坏死凋亡,出现急、慢性心衰。贫血后血管扩张、交感神经激活，同时RAAS系统及抗利尿激素激活，导致水钠潴留、肾血管收缩使肾功能受损。
4 结 语
目前对CRS 的病理生理学机制极尚未阐明。中医学较早地认识到了心肾的生理、病理关系。近年，西医学也逐步的对CRS的机制进行揭示。随着诊治水平的提高，借助中西结合的优势将会对CRS将会有更深的认知。
