股骨近端防旋髓内钉内固定治疗股骨粗隆间骨折疗效观察(1)
【摘 要】目的:對股骨近端防旋髓内钉内固定治疗股骨粗隆间骨折的临床效果进行分析和探讨。方法:我院选择2014年6月-2017年6月接收的82例股骨粗隆间骨折患者作为研究对象进行分析,将所有患者依据守手术方式进行分组,分成实验组和对照组。每组患者41例。其中,对对照组患采用股骨近端钢板或带锁髓内钉及动力髋内固定三种方式对比进行治疗,对实验组患者采用股骨近端防旋髓内钉进行治疗,然后对两组患者的术后并发症发生概率、治疗优良率以及手术时间、手术切口长度、住院时间、术后下床活动时间和骨折愈合时间等指标进行分析比较。结果:在实验组中,患者治疗优良率为95%,并发症发生概率为5%,而对照组患者治疗优良率为90%,并发症发生概率为22%,两组差异显著,P值<0.05,因而具有统计学意义。此外,实验组患者在手术时间、手术切口长度、住院时间、下床活动时间、以及骨折愈合时间等指标评估中显著优于对照组,P值<0.05,差异显著,具有统计学意义。结论:通过对股骨粗隆间骨折患者实施股骨近端防旋髓内钉内固定治疗,能够显著提高治疗优良率,降低患者术后出现并发症的概率,同时具有术中出血少、创伤小、固定牢靠、住院时间短以及愈合恢复快等优点,因而具有很高的临床应用价值。

Therapeutic effect of proximal femoral nail fixation for the treatment of femoral intertrochanteric fracture
Abstract Objective: To analyze and discuss the clinical effect of proximal femoral nail fixation for the treatment of intertrochanteric fractures. METHODS: Eighty-two patients with femoral intertrochanteric fractures who were received from June 2014 to June 2017 were selected as subjects. All patients were divided into experimental group and control group according to the way of surgery. There were 41 patients in each group. Among them, the control group was treated with the proximal femoral plate or the interlocking intramedullary nail and the dynamic intra-hip internal fixation. The experimental group was treated with the proximal femoral anti-rotation nail and then the two groups of patients. The prognosis probability, the excellent rate of treatment, the operation time, the length of the incision, the length of hospital stay, the time of postoperative ambulation and the time of fracture healing were analyzed and compared. RESULTS: In the experimental group, the excellent rate of treatment was 95%, and the probability of complication was 5%. The excellent rate of treatment was 90% in the control group and 22% in the complication. The difference between the two groups was significant. 0.05, thus statistically significant. In addition, the experimental group was significantly better than the control group in the evaluation of the operation time, the length of the incision, the length of hospital stay, the time of getting out of bed, and the time of fracture healing. The P value was <0.05, and the difference was statistically significant. Conclusion: The treatment of femoral intertrochanteric fractures with proximal femoral anti-rotation intramedullary nailing can significantly improve the rate of treatment and reduce the probability of postoperative complications, with less intraoperative bleeding and less trauma. It has the advantages of firm fixation, short hospital stay and quick recovery of healing, so it has high clinical application value., 百拇医药(龚克)
