短期胰岛素泵强化降糖治疗对2型糖尿病患者胱抑素C及同型半胱氨酸的影响(1)

     [摘要] 目的 评价短期胰岛素泵强化降糖治疗对2型糖尿病患者胱抑素C(Cys-C)及同型半胱氨酸(Hcy)的影响。方法 2017年9月—2018年9月间该院单纯2型糖尿病患者64例随机分为A组和B組,每组32例。A组短期胰岛素泵强化降糖治疗,B组预混胰岛素治疗,均治疗2周,并取同期该院健康体检人群40名为C组。观察治疗前后3组的空腹血糖(FPG)、餐后2 h血糖(2 hPBG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、空腹胰岛素(INs)、空腹C肽、Cys-C和Hcy水平。结果 FPG、2 hPBG、HbA1c、TC、TG、HDL-C、LDL-C、INS、空腹C肽、Cys-C和Hcy水平比较:A组治疗前后差异有统计学意义(P<0.05);治疗前后B组除TG,HDL-C和LDL-C水平差异无统计学意义(P>0.05),其余指标(P<0.05);治疗前3组均差异有统计学意义(P<0.05),治疗后AB组以及BC组比较差异有统计学意义(P<0.05),治疗后A组除FPG、2 hPBG、HbA1c和Cys-C水平与C组差异有统计学意义(P<0.05),其余指标均与C组差异无统计学意义(P>0.05)。结论 对于2型糖尿病患者,短期胰岛素泵强化降糖治疗可明显降低患者FPG、2hPBG、HbA1c、TC、TG、LDL-C、INS、Cys-C和Hcy水平,提高HDL-C和空腹C肽水平,缓解患者糖尿病肾脏病变及微血管病变的影响,更好的指导临床工作。

    [Abstract] Objective To evaluate the effects of short-term insulin pump intensive glucose-lowering therapy on cystatin C (Cys-C) and homocysteine (Hcy) in patients with type 2 diabetes mellitus. Methods From september 2017 to Semtember 2018 64 patients with type 2 diabetes mellitus in our hospital were randomly divided into group A and group B, with 32 cases in each group. Group A short-term insulin pump intensive hypoglycemic therapy, group B premixed insulin treatment, were treated for 2 weeks, and 40 patients in our hospital for physical examination in the same period were group C. Fasting blood glucose (FPG), postprandial 2-hour blood glucose (2 hPBG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high-density lipoprotein (HDL-C), Low density lipoprotein (LDL-C), fasting insulin (INs), fasting C-peptide, Cys-C and Hcy levels. Results The levels of FPG, 2 hPBG, HbA1c, TC, TG, HDL-C, LDL-C, INS, fasting C peptide, Cys-C and Hcy were significantly different between the two groups (P<0.05). There were no significant differences in the levels of TG, HDL-C and LDL-C in group B (P>0.05), and other indicators were also significantly different (P<0.05). There were significant differences in the three groups before treatment (P<0.05). There were statistically significant differences between the AB group and the BC group after treatment (P<0.05). After treatment, the levels of FPG, 2 hPBG, HbA1c and Cys-C in group A were significantly different from those in group C (P<0.05). There was no significant difference between the two groups (P>0.05). Conclusion For patients with type 2 diabetes, short-term insulin pump intensive hypoglycemic therapy can significantly reduce the levels of FPG, 2 hPBG, HbA1c, TC, TG, LDL-C, INS, Cys-C and Hcy, and increase the levels of HDL-C and fasting C-peptide, alleviate the effects of diabetic nephropathy and microvascular disease in patients, and better guide clinical work., 百拇医药(刘效荣 侯娟 吴燕 张计划 刘彦娥)
