胆囊结石合并糖尿病患者围手术期实施个性化护理的效果分析(1)
[摘要] 目的 对围术期个性化护理在胆囊结石合并糖尿病患者护理中应用的效果进行观察。 方法 选取2017年3月—2018年2月60例胆囊结石合并糖尿病患者，平均分为观察组(n=30，常规护理)与对照组(n=30，围术期个性化护理)，对比两组护理效果。 结果 空腹血糖、餐后2 h血糖对比，护理后观察组均低于对照组(P<0.05);观察组术后排气时间、进食时间、下床活动时间、住院时间及VAS评分均低于对照组(P<0.05);观察组术后并发症发生率为6.67%，低于对照组的20.00%(P<0.05)。结论 围术期实施个性化护理的应用，能够使胆囊结石合并糖尿病患者血糖水平有效控制，同时促使患者术后胃肠功能、机体功能尽早恢复，减轻患者术后疼痛感，减少术后相关并发症的发生，有利于患者术后康复。

[Abstract] Objective To observe the effect of perioperative personalized nursing in the care of patients with gallstones complicated with diabetes. Methods Sixty patients with cholecystolithiasis and diabetes from March 2017 to Febreuary 2018 were selected and divided into observation group (n=30， routine care) and control group (n=30， perioperative personalized care)， and the nursing effects of the two groups were compared. Results Compared with fasting blood glucose and 2h postprandial blood glucose， the observation group after nursing was lower than the control group (P<0.05); the postoperative exhaust time， eating time， time to get out of bed， hospitalization time and VAS score of the observation group were lower than the control group(P<0.05); the postoperative complication rate in the observation group was 6.67%， lower than that in the control group， 20.00%(P<0.05. Conclusion The application of personalized care during the perioperative period can effectively control the blood glucose levels of patients with gallbladder stones and diabetes， and promote the early recovery of gastrointestinal function and body function of patients after surgery， reduce patients' postoperative pain， and reduce postoperative complications. The occurrence of is beneficial to the postoperative recovery of patients.

近年來，随着社会经济的发展，人们的饮食、生活习惯发生极大改变，也导致胆囊结石、糖尿病等疾病的发病率不断升高，在胆囊结石治疗中，腹腔镜胆囊切除术治疗的患者数量也增多[1]。由于糖尿病、胆囊结石疾病在老年人群中比较多发，患者因年龄较大，伴有基础疾病较多，在手术治疗胆囊结石后，术后恢复时间也比较长，且容易引起诸多并发症的发生，对恢复效果带来影响。因此，对胆囊结石合并糖尿病患者实施外科手术治疗期间，需从护理角度入手，为患者从心理、生理、社会等方面提供个体化、针对性的护理措施，对围术期相关并发症积极预防，减少并发症发生，使患者术后能够尽早康复。该次针对2017年3月—2018年2月期间60例胆囊结石合并糖尿病患者进行研究，观察腹腔镜胆囊切除术围术期个性化护理应用效果，现报道如下。
1 资料与方法
1.1 一般资料
