建立中医男性不育症三级预防体系(1)

【摘要】本文以中医治未病思想，提出了“建立中医男性不育症三级预防体系”学说，针对不同人群制定相应的预防保健措施。一级预防(病因预防)是针对致病因素的预防措施。着重于保持男性良好的生殖能力，首先要安神，强调摄生；其次养精，保持良好的生活方式，戒烟、戒酒，饮食有节制，生活起居有规律；第三调食，要“饮食有节，起居有常，不妄劳作”， 食有助于精子健康成长的食物；第四做好育前体检。保证男性生育者有良好的生殖能力。二级预防(临床前期预防)，是在不育症的临床前期作好“三早”预防措施(即早期发现、早期诊断、早期治疗)。早期发现的具体方法有普查(筛检)、定期健康检查、高危人群重点项目检查等。对筛检阳性者必须进行进一步确诊，定期健康检查除了常规体检项目，育前检查更侧重于检测生殖器官、精液常规和体内激素水平的健康状况。发现有传染病和遗传病家族史者为高危人群，重点项目检查包括，常见的遗传性疾病检查如地中海贫血、G6PD缺乏症和染色体检查。常见的传染病检查包括精液或前列腺衣原体、支原体、淋球菌等微生物感染的性病的相关检测，肝炎、梅毒、艾滋病的相关检测。发现阳性者，必须进行进一步确诊，对确诊的病人再进行必要的治疗。三级预防(临床预防)，对已患不育症者及时治疗。建立以男科医院或生殖医学为中心，男科门诊为辅，其他学科配合的不育症临床预防治疗网。针对病因进行治疗，发挥中医、西医、中西医结合各自治疗不育症的优势，优势互补。中医治疗不育症诊断以中医四诊和现代检测相结合，治疗以中医辨病和辨证相结合，以中为主，中西医结合。做到治疗男性不育症防病于未然。
