【摘要】目的:通过对甘肃省酒泉市阿克塞县地区新生儿进行听力筛查情况调查,获得本地区听力障碍的基本消息。探讨为真正做到早期发现有听力障碍的,耳道和中耳腔有异常的,早期新生儿听力传导和神经系统发育不成熟的、并能给予及时干预 ,来进一步提高新生儿听力筛查质量。方法:采用手持式耳声发射听力筛查仪-瞬态诱发性耳声发射【TEOAE】进行筛查,通过初筛.复筛对新生儿听力障碍得到了早发现.早诊断.早干预。方法:采用对比性分析,通过对不同民族.性别.耳别的通过率和复筛率的统计分析。结果:初筛率;96%,初筛未通过率:9%,复筛率:95%,复筛未通过率:1%,最终确诊率0%,假阳性率:9%。跟家族耳聋史,听力筛查的时间,外耳和中耳的因素,测试探头的放置,测试探头的校对和清洁等有关,与是何民族的新生儿无关。结论:提高新生儿听力筛查的质量,才能体现新生儿听力筛查项目普及的意义和价值。

【Abstract】Objective:Through carries on the hearing screening situation investigation to the Gansu Province Jiuquan Akesai county area newborn, obtains this area hearing disorder the basic news. The discussion to achieve the early time to discover that truly has the hearing disorder, the meatus auditorius and the middle ear cavity has exceptionally, the early newborn hearing conduction and the nervous system growth not mature, and can give the prompt intervention, further improves the newborn hearing screening quality. Method: Uses the handhold ear sound launch hearing screening meter - transient state causality ear sound launch[TEOAE]Carries on screening, through initially sieves. The affination obtained early the discovery to the newborn hearing disorder. Early diagnosis. Early intervention. Method: Uses the relative property analysis, passes to the different nationality. Sex. Ear other capacity and affination rate statistical analysis. Finally: Initially sieving rate; 96%, initially do not sieve the capacity: 9%th, affination rate: 95%, affination not capacity: 1%th, final diagnosis rate 0%, false positive rate: 9%. With family deafness history, hearing screening time, external ear and middle ear's factor, test probe's laying aside, test probe's proofreading and clean and so on related, with is what nationality's newborns has nothing to do with. Conclusion: Enhances newborn hearing screening the quality, can manifest the newborn hearing screening project popular significance and the value.

1对象与方法
1.1对象:筛查对象是2010年1月至10月出生的活产新生儿94例,其中哈族38例,回族2例,汉族46例{地域及医疗条件限制筛查时间并且是全额免费走向社区家庭服务}。除上级医院初筛通过的外,其余均在家长知情同意书签字后进行测试的。

1.2筛查方法:由经过上岗培训专人负责新生儿听力筛查的医务人员,与家属签订“新生儿听力筛查知情同意书”通知家属。如果拒绝筛查还要签拒绝原因知情签名。采用手持式耳声发射听力筛查-瞬态诱发性耳声发射【TEOAE】仪对出生后10天内的新生儿在安静的房间内测试,受试着均在安静或睡眠的状态下进行,用棉签清洁耳道,测试中拉直耳道,左右耳随机开始,将仪器探头紧密塞于外耳道。测试结果告知产妇并将测试结果打印让产妇签字确认。初筛未通过的需要在出生后42天复筛,复筛未通过的发转诊单到省级医院复诊。
1.3 筛查标准:耳声发射仪显示通过pass和不通refer.pass意味着测试通过(说明耳蜗功能及听骨链、鼓膜及外耳道等在内的功能正常)。refer表明受试者可能有听力损失。在排除外在因素重复检测仍未通过,42天复筛。TEOAE的标准:伪迹率《20%,刺激稳定率》80%。
1.4 结果:初筛率:96%,初筛未通过率:9%,复筛率:95%,复筛未通过率:1%,最终确诊率0%,假阳性率:9%。其中一回族的早产儿在42天,三个月,六个月后复筛才通过。, http://www.100md.com(张 英)
