摘要：对中国及川东北地区老龄化现状进行剖析，从川东北地区社区老年人健康状况及对社区护理的需求入手，以老年人的社区护理服务模式：建立社区老年人档案、实行“医养融合”、建立老年护理院、建立家庭病床、访视护理和设立老年日托护理中心等为重点，展示川东北地区老年社区护理服务现状，分析出社区老年护理存在的问题，对如何实行区域性的老年社区护理提出了相应的建议。
