化脓性脑膜炎血液中性粒细胞CD64的测定及临床意义(1)

     [摘要] 目的:探讨中性粒细胞CD64、肿瘤坏死因子-α(TNF-α)水平在化脓性脑膜炎中的诊断价值。方法:选取化脓性脑膜炎患者30例及病毒性脑炎患者36例为研究对象,非中枢神经系统疾病患者32例作为对照组,采用流式细胞术测定各组血液中性粒细胞CD64的表达,采用双抗体夹心酶联免疫吸附法测定各组血液中TNF-α的含量。结果:化脓性脑膜炎组中性粒细胞CD64的表达水平显著升高,分别与病毒性脑炎组和对照组比较差异有显著性(P<0.01),化脓性脑膜炎组TNF-α的水平显著升高,分别与病毒性脑炎组和对照组比较差异有显著性(P<0.01)。CD64诊断化脓性脑膜炎的敏感度为87%,特异度为88%;TNF-α诊断化脓性脑膜炎的敏感度为66%,特异度为62%。结论:化脓性脑膜炎患者外周血中性粒细胞CD64的表达明显增高,提示中性粒细胞CD64可以作为诊断化脓性脑膜炎的可靠的实验室指标。

    [Abstract] Objective: To explore the diagnostic value of neutrophil CD64 and tumor necrosis factor α (TNF-α) levels in purulent meningitis. Methods: 30 patients with purulent meningitis and 36 patients with viral encephalitis were selected as subjects, and 32 patients with non-central nervous system diseases served as control, then flow cytometry was used to detect the expression of neutrophil of CD64 in each group, and double-antibody sandwich enzyme-linked immunosorbent assay was used to determine TNF-α content in each group. Results: The group neutrophil CD64 expression level of purulent meningitis was significantly higher, and showed significant difference respectively compared with that of viral encephalitis group and control group (P<0.01), the level of TNF-α of purulent meningitis group was significantly higher, and showed significant difference respectively compared with that of viral encephalitis group and control group (P<0.01). The sensitivity CD of 64 in diagnosing purulent meningitis was 87%, and the specificity was 88%. The sensitivity of TNF-α in diagnosing purulent meningitis was 66%, and the specificity was 62%. Conclusion: The neutrophil CD64 expression of patients with purulent meningitis was significantly increased, suggesting that neutrophil CD64 could serve as a reliable laboratory indicators of diagnosing diagnosis of purulent meningitis ......
