[摘要] 目的:促进儿科门诊抗菌药物合理应用。制订合理使用抗菌药物的对策。方法:随机抽查解放军第一七五医院1周儿科门诊处方409张,统计分析抗菌药物使用情况。结果:儿科门诊抗菌药物使用率为53.55%,青霉素类、头孢菌素类、大环内酯类、硝基咪唑类和林可霉素类应用最多。结论:解放军第一七五医院儿科门诊抗菌药物的使用基本合理,但也存在一些问题。制订合理的对策,促进合理用药。

Investigation and analysis of the rational uses of antibiotics for outpatient in pediatrics

[Abstract] Objective:To promote the rational uses of antibiotics for outpatient in pediatrics and to make reasonable strategies for them. Methods: Through the random survey of 409 outpatient prescriptions in pediatrics of the 175th Hospital of PLA for one week, the uses of antibiotics were analyzed by statistical methods. Results: The usage of prescriptions of antibiotics for outpatient in pediatrics accounted for 53.35 percent, in which Penicillins, Cephalosporins, Macrolides, Nitroimidazoles, Lincomycins were used mostly. Conclusion: The utilization of antibiotics for outpatient in pediatrics of the 175th hospital of PLA is reasonable the whole. But there are still some problems. Making reasonable strategies, to the promote the rational applications of antibiotics.

近几年来,抗菌药物已经成为门诊儿科医生应用最广泛,品种最多,进展最快的一种药物。为了更好地了解解放军第一七五医院门诊儿科抗菌药物应用情况,促进合理用药,根据《抗菌药物临床应用指导原则》,随机抽查解放军第一七五医院1周儿科门诊处方,对其使用的抗菌药物进行统计、分析,并结合存在问题,提出合理应用抗菌药物的建议。
1资料与方法
1.1 一般资料
从解放军第一七五医院2008年8月25日～8月31日1周门诊儿科处方中,随机抽取409张处方。
1.2方法
分别就年龄、性别、诊断、药物使用品种数、抗菌药物使用品种数、用法、疗程、联合用药等进行统计。
2结果
2.1一般情况
409张处方中,男 138例,女271 例;年龄最小4 d,最大14岁,平均(2.83±2.35)岁,处方平均用药品种数平均值3.27元,处方平均金额为17.48元。
2.2 抗菌药物使用情况
2.2.1 基本情况使用抗菌药物处方 219 张,占53.55%;抗菌药物处方平均金额为10.39元,抗菌药物的总金额占处方总金额的35.41%。
2.2.2 疾病分布处方中抗菌药物应用较多的分别是:上呼吸道感染182例(共322例),占56.52%,下呼吸道感染29例(共48例),占60.42%,消化系统感染8例(共39例),占20.51%。
2.2.3 使用种类、品种及使用频率共涉及抗菌药物5大类13个品种。使用的抗菌药物及其使用率见表1,使用率居前7位的抗菌药物及其在上、下呼吸道和消化系统疾病中的使用例数见表2。
2.2.4 联合应用处方平均使用抗菌药物品种数0.537 9。单用1种抗菌药物的有218张,占53.30%;联用2种抗菌药物的有1张,占0 ......
