逐步后退法与常规法在根管预备中的疗效分析(1)

[摘要] 目的:通过对临床上感染根管的治疗,比较逐步后退法与常规法的疗效,从而为临床上感染根管的治疗提供参考。方法:将临床感染根管病例362例随机分为两组,试验组(S组,n=185)用逐步后退法预备根管,对照组(R组,n=177)用常规法预备根管。根管治疗完成后随即拍片观察,应用SPSS 13.0统计软件包对数据进行处理。结果:对于弯曲根管,对照组出现根管预备中常见的缺陷明显高于试验组;对于普通根管(较直根管),两组比较无显著性差异。结论:逐步后退法在弯曲根管预备中优于传统常规法;常规法适应证选择面很窄,只适用于直根管。

Clinical analysis of step-back technique and the conventional method in the root canal preparation

[Abstract] Objective: To provide reference for the treatment of the infected root canal through the comparison on the clinical effect between the step-back technique and routine preparation technique. Methods: 362 cases of patients were divided into two groups randomly. 185 cases were prepared by the step-back technique (group S) while 177 cases were prepared by the routine preparation technique (group R).The effect was recorded right now. The results were assessed by SPSS 13.0 software package. Results: The curative rate of group S was higher than that of group R for the curving root canals, there was no difference between the two groups for normal root canals; group S was significantly better than group R on the incidence and degree of the post-operative pain. Conclusion: The step-back technique is better than the routine preparation technique. Routine preparation technique is only suit for straight root canals.

根管治疗是牙髓病和根尖周病首选的方法,它包括彻底清理根管内炎症牙髓和坏死物质,扩大成形根管,并对根管进行适当消毒,最后严密充填根管[1]。其中,根管预备是根管治疗术的关键步骤,根管治疗术成功与否很大程度上取决于根管预备的质量[2]。临床常用的根管预备方法主要有2种,即常规法和逐步后退法。本文通过对比2种方法在不同根管条件下预备根管时是否出现缺陷,为临床根管治疗提供参考。
1 资料与方法
1.1 一般资料
选取2006年2月~2008年2月在口腔门诊就诊的临床诊断为牙髓病、根尖周病患者362例,牙位为前牙和下颌磨牙,其中,男200例,女162例;年龄18~29岁76例,30~60岁222例,61~79岁64例。随机分为两组,试验组185例使用不锈钢扩孔锉逐步后退法(step-back technique),简称S组,其中普通根管(即:根管通畅,牙根弯曲度<30°的根管)89例,弯曲根管(牙根弯曲度>30°的根管)67例,狭窄根管(15号以上的扩大针不能进入的根管)29例。对照组177例使用不锈钢扩孔钻和根管锉交替使用,常规法(routine preparation technique)预备根管,简称R组,其中,普通根管86例,弯曲根管65例,狭窄根管26例 ......
