[摘要] 目的：观察无创机械通气治疗慢性阻塞性肺疾病(COPD)并Ⅱ型呼吸衰竭的疗效，并总结治疗经验，提高应用无创机械通气的治疗水平。方法：对36例老年COPD并Ⅱ型呼衰患者在常规治疗(抗感染、平喘、祛痰等)基础上给予无创机械通气治疗，观察治疗前后的临床症状、体征及动脉血气变化。结果：经无创机械通气治疗后，36例患者中有30例患者明显好转，有效率为83%。结论：无创机械通气治疗COPD并Ⅱ型呼吸衰竭疗效较好。在无创机械通气的使用中，合适的适应证选择、心理干预、参数调节及气道管理对疗效影响较大。

Experience of using non-invasive mechanical ventilation on patients due to type Ⅱ respiratory failure of chronic obstructive pulmonary disease

[Abstract] Objective：To observe the efficacy of non-invasive mechanical ventilation in treatment of COPD with Type Ⅱ respiratory failure，summarize the treatment experience and improve the treatment levels of non-invasive mechanical ventilation. Methods：36 elderly COPD patients with type Ⅱ respiratory failure were given non-invasive mechanical ventilation based on conventional therapy(anti-infection, relieving asthma, eliminating phlegmt, etc). Observation was made on the clinical symptoms and changes of arterial blood gas in pre/post-therapy. Results：After therapy of non-invasive mechanical ventilation, thirty patients(83%) got benefit from the treatment. Conclusion：Non-invasive mechanical ventilation has good effection COPD cases with type Ⅱ respiratory failure when using non-invasive mechanical ventilation, the appropriate indications of therapy, psychological intervention, parameter adjustment and airway management have a greater impact on the efficacy.

慢性阻塞性肺疾病(COPD)是一种具有气流受限为特征的疾病，气流受限不完全可逆并进行性进展[1，2]。随着疾病迁延进展，在COPD急性加重期常合并呼吸衰竭，临床上常规抗感染、解痉、祛痰等治疗常难以达到满意疗效，对于重症患者机械通气是抢救治疗的重要方法。双水平正压通气(BIPAP)呼吸机因其无创性、快捷方便、操作简单等特点，已广泛应用于呼衰患者救治。现就我科2009年1～10月间收治的36例COPD急性发作期并Ⅱ型呼吸衰竭患者使用BIPAP无创呼吸机治疗情况报道如下：
1 资料与方法
1.1 一般资料
选择我院2009年1～10月收治的COPD合并Ⅱ型呼吸衰竭患者36例，男24例，女12例；年龄62～78岁，平均(69.0±7.2)岁。所选患者均符合我国2007年修订的《慢性阻塞性肺疾病诊治指南》中的COPD诊断标准[1]，均有程度不等的咳嗽、咳痰、呼吸困难、嗜睡等症状，治疗前血气分析，动脉血二氧化碳分压(PaCO2)>50 mm Hg，动脉血氧分压(PaO2)<60 mm Hg；常规予抗感染、解痉、平喘、吸氧等治疗，患者临床症状、体征、血气分析未能改善。
1.2 治疗方法
常规治疗基础上，应用美国伟康公司生产的BIPAP-VISION无创呼吸机经口鼻面罩通气治疗；通气模式为S/T模式；通气参数：备用通气频率12～20次/min；IPAP为8～20 cm H2O ......
