[摘要] 目的:探讨机械通气治疗慢性肺源性心脏病呼吸衰竭的临床疗效。方法:回顾性分析我院重症监护病房80例危重肺源性心脏病呼吸衰竭患者的临床资料,其中,41例采取机械通气治疗为观察组,39例未采取机械通气治疗为对照组,观察比较两组的治疗效果。结果:观察组患者治疗总有效率为92.68%,对照组治疗总有效率为74.36%,观察组疗效显著优于对照组(P<0.05)。观察组患者PaO2、PaCO2、pH及肺功能改善程度显著高于对照组,两组比较,差异均有统计学意义(均P<0.05)。结论:对危重肺源性心脏病呼吸衰竭患者采取机械通气,可迅速纠正患者低氧血症及高碳酸血症,在改善患者症状的同时,可显著减少多脏器功能不全的发生,对于此类患者在常规内科治疗基础上,应合理使用机械通气治疗,以争取抢救时间,为后续治疗提供有利条件。

Clinical efficacy of mechanical ventilation in the treatment of critical patients with cor pulmonale with respiratory failure

[Abstract] Objective: To explore the clinical efficacy of mechanical entilation in the treatment of chronic cor pulmonale with respiratory failure. Methods: The clinical data of the 80 cases with cor pulmonale with respiratory failure in the intensie care unit of our hospital were analyzed retrospectively, 41 cases were treated by mechanical entilation as the observation group, 39 cases failed to take mechanical entilation as the control group, and the efficacy of the two groups were compared. Results: The total effective rate of the observation group was 92.68%, the control group was 74.36%, the efficacy of the observation group was significantly higher than that of the control group (P<0.05). The improvement of the PaO2, PaCO2, pH andlung function of the observation group were significantly higher than those of the control group (all P<0.05). Conclusion: If mechanical entilation is taken for the critical patients with cor pulmonale with respiratory failure, the hypoxia and hypercapnia could be taken corrected quickly, the symptoms can be improved and the multiple organ dysfunction are significantly reduced. The mechanical entilation should be reasonably used for such patients based on the conventional medical treatment to rescue time and provide favorable conditions for the follow-up treatments.
