心肌肌钙蛋白Ⅰ水平在慢性心力衰竭患者中检测的价值分析(1)

     [摘要] 目的 探讨心肌肌钙蛋白I(cTnI)水平在慢性心力衰竭患者中检测的临床价值。 方法 对114例慢性心力衰竭(CHF)患者cTnI水平进行检查,并分析cTnI与心功能分级、左室射血分数以心脏结构的关系。 结果 心功能Ⅳ级的CHF患者cTnI水平明显高于心功能Ⅲ级和心功能Ⅱ级患者的cTnI水平(t = 13.23,9.62,P < 0.01),心功能Ⅲ级患者的cTnI水平明显高于心功能Ⅱ级患者的cTnI水平(t = 7.54,P < 0.01),并且LVEF<40%的患者cTnI水平明显高于LVEF 40%~50%、LVEF>50%患者cTnI水平,差异有高度统计学意义(t = 18.34,13.86,P < 0.01),LAD和LVEDd在CTnI升高组患者中明显高于CTnI正常组患者(t = 6.54,7.45,P < 0.01),而LVS、LVPW在CTnI正常组中显著高于CTnI升高组,差异有高度统计学意义(t = 16.54,8.78,P < 0.01)。 结论 CTnI能够反映CHF患者心功能分级、左室射血及心室重构情况,值得临床推广应用。

    The value analysis of cardiac troponin I levels detection in patients with chronic heart failure

    [Abstract] Objective To investigate clinical value of cardiac troponin I (cTnI) levels in patients with chronic heart failure. Methods CTnI levels of 114 patients with chronic heart failure were checked. The correlation among cTnI and cardiac function, left ventricular ejection fraction and cardiac structure were analyzed. Results The cardiac function class Ⅳ in patients with CHF cTnI levels were significantly higher than class Ⅲ and class Ⅱ patients (t = 13.23, 9.62, P < 0.01), class Ⅲ patients′ cTnI level were significantly higher than that of class Ⅱ patients (t = 7.54, P < 0.01), and LVEF<40% patients′ cTnI levels were significantly higher than that of LVEF 40%-50%, LVEF>50% patients (t=18.34, 13.86, P < 0.01). LAD and LVEDd in CTnI elevated group were significantly higher than those of the normal group (t = 6.54, 7.45, P < 0.01), and LVS, LVPW in normal group was significantly higher than elevated group, the difference was statistically significant (t = 16.54, 8.78, P < 0.01). Conclusion cTnI level can reflect cardiac function in patients with CHF, left ventricular ejection conditions and ventricular remodeling situation. It's worthy of promotion and application. ......
