陀螺旋转式钴60立体定向放射系统治疗食管癌的疗效观察(1)

     [摘要] 目的 观察陀螺旋转式钴60立体定向放射系统(陀螺刀)治疗食管癌的近期疗效和急慢性毒副反应。 方法 采用国产陀螺旋转式钴60立体定向放射系统治疗食管癌120例,全组处方剂量4 200~4 800 cGy,中位处方剂量4 500 cGy,350 cGy/次,5次/周,对其近期疗效及急慢性毒副反应进行评价。 结果 ①1年局部控制率为72.50%,1年总生存率为79.25%,1年无瘤生存率为71.18%。②全组患者完全缓解81例(67.50%),部分缓解38例(31.67%),无缓解1例(0.83%),肿瘤总有效率为99.17%。③急性放射性肺损伤0级101例(84.16%),1级19例(15.83%),无2、3、4级发生;晚期放射性肺损伤0级112例(93.33%),1级8例(6.67%),无2、3、4级发生;急性放射性食管损伤0级20例(16.67%),1级98例(81. 67%),2级2例(1.7%),无3、4级发生。晚期放射性食管损伤0级101例(84.17%),1级17例(14.17%),2级2例(1.67%),无3、4级发生。 结论 陀螺刀治疗食管癌初期疗效较好,急慢性放射性食管和肺损伤发生率较低,是一种有效的局部治疗方法,其远期疗效有待进一步观察。

    [Abstract] Objective To observe the therapeutic efficacy acute and chronic toxicity of cobalt-60 gyro rotating stereotactic radiotherapy system (gyro knife) in the treatment of esophageal cancer. Methods 120 esophageal cancer patients were treated with gyro knife. The prescription doses of the group were 4 200 to 4 800 cGy and the median prescribed dose was 4 500 cGy. The dose was delivered at 350 cGy each time for 5 times a week. The efficacy and acute and chronic toxicity side effects were evaluated. Results ①1-year local control rate was 72.50%. 1-year overall survival rate was 79.25%. And 1-year survival rate was 71.18%. ② The treatment achieved complete remission in 81 patients (67.50%), partial remission in 38 cases (31.67%) and no relief in 1 case, with overall effective rate of 99.17%. ③ Grade 0 acute radiation-induced lung injury occurred in 101 cases (84.16%) and grade 1 injury occurred in 19 cases (15.83%), with no reports of grade 2, 3 or 4-level acute lung injury; grade 0 late radiation-induced lung injury occurred in 112 cases (93.33%) and grade 1 injury occurred in 8 cases (6.67%), with no reports of grade 2, 3 or 4 late lung injury; grade 0 acute radiation-induced esophageal injury occurred in 20 cases (16.67%), grade 1 injury occurred in 98 cases (81.67%) and grade 2 injury occurred in 2 cases (1.7%), with no reports of grade 3 or 4 injuries: late radiation-induced esophageal injury occurred in grade 0 was 101cases (84.17%), grade 1 injury occurred in 17 cases (14.17%), grade 2 injury occurred in 2 cases (1.67%), with no reports of grade 3 and 4 injury. Conclusion Gyro knife is an effective treatment for early esophageal cancer with low incidence of acute and chronic radiation-induced esophageal and lung injury. it is also an effective local treatment.
