超声检查和钼靶X线摄片对乳腺疾病的诊断价值(1)

     [摘要] 目的 探讨超声和钼靶X线摄片对乳腺疾病的诊断价值。 方法 回顾性分析2010年6月~2014年6月河北省衡水市哈励逊国际和平医院普外科326例乳腺疾病超声和钼靶X线摄片检查结果,并与病理结果进行比较。 结果 326例患者中发现357个乳腺病灶,经病理证实其中良性病变288个,恶性病变69个;超声检查发现良性病变的准确率为89.58%,误诊率为10.42%,恶性病变的准确率为84.06%,误诊率为15.94%;钼靶X线摄片检查发现良性病变的准确率为82.99%,误诊率为17.01%,恶性病变的准确率为75.36%,误诊率为24.64%;超声和钼靶X线摄片联合,发现良性病变的准确率为96.53%,误诊率为3.47%,恶性病变的准确率为88.41%,误诊率为11.59%。不同检测方法准确率及误诊率比较,差异均有统计学意义(P < 0.05)。 结论 超声检查的准确率高于钼靶X线摄片,误诊率低于钼靶X线摄片,超声和钼靶X线摄片联合检查的准确率高于任何一项单独检查的准确率,误诊率低于任何一项单独检查的误诊率。

    [Abstract] Objective To explore the diagnostic value of the mammography and ultrasound for breast disease. Methods In Harrison International Peace Hospital of Hengshui, from June 2010 to June 2014, the image findings of the mammography and ultrasound for 326 patients with breast tumor were retrospectively analyzed, which were compared with the pathology results. Results There were 357 breast tumors in 326 patients, in which 288 cases were benign and 69 cases were malignant by the pathology test. The accuracy rate and misdiagnosis rate in benign breast tumors tested by ultrasound was 89.58%,10.42% respectively, and the accuracy rate and misdiagnosis rate in malignant breast tumors tested by ultrasound was 84.06%, 15.94% respectively; the accuracy rate and misdiagnosis rate in benign breast tumors tested by mammography was 82.99%, 17.01% respectively, and the accuracy rate and misdiagnosis rate in malignant breast tumors tested by mammography was 75.36%, 24.64% respectively; the accuracy rate and misdiagnosis rate in benign breast tumors tested by ultrasound and mammography was 96.53%, 3.47% respectively, the accuracy rate and misdiagnosis rate in malignant breast tumors tested by ultrasound and mammography was 88.41%,11.59% respectively. The differences of accuracy rate and misdiagnosis rate in different test methods were statistical significance (P < 0.05). Conclusion The accuracy rate of ultrasound test is higher than that of mammography test, the misdiagnosis rate of ultrasound test is lower than that of mammography test; the accuracy rate of the combined application of the two methods is higher than other one method, the misdiagnosis rate of the combined application of the two methods is lower than the other one method.

    乳腺疾病是女性常见病、多发病,其中乳腺囊性增生病、乳腺纤维腺瘤、乳腺囊肿、乳腺癌是常见乳腺病,近年来,乳腺癌已成为女性的第一位恶性肿瘤,且呈低龄化趋势[1],如何早期诊断与治疗乳腺癌,是临床医生的难题之一。总结了河北省衡水市哈励逊国际和平医院普外科2010年6月~2014年6月资料齐全的326例乳腺疾病超声和钼靶X线摄片影像表现,并与病理结果对比分析,现报道如下: (谷云芝等)
