老年高血压患者动态血压与动脉硬化的相关性(1)

     [摘要] 目的 探讨老年原发性高血压患者动态动脉硬化指数(AASI)与动脉脉搏波传导速度以及颈动脉粥样硬化的相关性,评价AASI对老年高血压患者早期动脉硬化的预测价值。 方法 选择2012年11月~2014年5月南通大学附属医院分院老年原发性高血压患者104例。所有患者均给予颈动脉超声检查和24 h动态血压监测,并用臂踝脉搏波传导速度(baPWV)自动测量装置测定baPWV。根据颈动脉内膜中层厚度(IMT)分为IMT正常组(n=28)、IMT增厚组(n=42)和斑块形成组(n=34)。根据动态血压参数按照文献方法计算AASI,比较三组的动态血压指标、AASI及baPWV,并对患者的动态血压参数、AASI、baPWV与颈动脉IMT的关系进行相关性分析。 结果 ①24 h脉压(24 hPP)在IMT增厚组与正常组比较差异无统计学意义(P > 0.05),但在IMT增厚组与斑块形成组比较,差异有高度统计学意义(P < 0.01);②AASI、baPWV在IMT正常组、IMT增厚组与斑块形成组比较,差异均有高度统计学意义(P < 0.01);③Pearson相关分析显示,颈动脉IMT与AASI及baPWV均呈正相关(r=0.604、0.587,P < 0.01),AASI与baPWV呈正相关(r=0.0567,P < 0.01)。 结论 24hPP、AASI以及baPWV均与老年高血压患者的颈动脉粥样硬化关系密切,但AASI是更简易的评价早期动脉硬化的指标。

    [Abstract] Objective To investigate the correlation between the ambulatory arterial stiffness index (AASI) and pulse wave velocity (PWV), and carotid atherosclerosis in elderly patients with essential hypertension, and to evaluate the value of AASI on early arteriosclerosis prediction. Methods A total of 104 elderly patients with essential hypertension underwent carotid color doppler ultra sonography and 24-hour ambulatory blood pressure monitoring (ABPM). BaPWV was measured using validated automatic devices. According to carotid intima-media thickness (IMT), the patients were divided into the normal IMT group (n=28), the thickened IMT group (n=42) and the plaque formation group (n=34); according to 24-hour ABPM, AASI were calculated using the methods described in refferences; differences in the level of ABPM, AASI and baPWV between the three groups were compared and the relationship of AASI with baPWV and carotid IMT was analyzed. Results ①The difference between the thickened IMT and normal IMT group was not significant (P > 0.05), but it is significant between the thickened IMT and plaque formation group (P < 0.01). ② In the thickened IMT group and the plaque formation group, AASI as well as levels of baPWV were higher than those in the IMT normal group (P < 0.01). The difference between the thickened IMT group and the plaque formation group was also significant (P < 0.01). ③ Pearson correlation analysis showed that carotid IMT was positively correlated with AASI (r=0.604, P < 0.01) and baPWV (r=0.587, P < 0.01). AASI and baPWV also had the positive correlation (r=0.567, P < 0.01). Conclusion AASI and baPWV are closely correlated with the severity of the carotid atherosclerosis in elderly patients with EH. AASI is a good index for evaluating early arteriosclerosis because of its easiness. (袁永梅 孙敏敏 杭晓英)
