锁定板固定锁骨骨折生物力学测试(1)
[摘要] 目的 比较锁定钢板与重建钢板的生物力学特性。 方法 将2009年1月~2015年3月通过民政部门、医院、福利院、遗体捐献等途径获得的9具新鲜成人尸体(锁骨标本9对),随机分为重建钢板组与锁定钢板组,各9根锁骨标本。通过轴向压缩实验、三点弯试验、扭转实验比较两组生物学特性。 结果 在各级载荷点,锁定钢板组应变值及位移值均小于重建钢板组,差异有统计学意义(P < 0.05),载荷越大差异越明显。600 N载荷作用下,锁定钢板组轴向刚度值、应力遮挡率均显著优于重建钢板组(P < 0.05)。在各级载荷点,锁定钢板组挠度值均小于重建钢板组,差异有统计学意义(P < 0.05),载荷越大差异越明显;在20 N·m载荷作用下,锁定钢板组板侧、对侧加载弯曲刚度均显著大于重建钢板组(P < 0.05)。在各级扭矩作用下,锁定钢板组扭转角度值均小于重建钢板组,差异有统计学意义(P < 0.05),扭矩越大差异越明显;在5 N·m扭矩作用下,锁定钢板组扭转刚度显著大于重建钢板组(P < 0.05)。 结论 对于锁骨骨折内固定患者,锁定钢板生物力学特性优于重建钢板。

[Abstract] Objective To compare the biomechanical characteristics of the locking plate and the reconstruction plate. Methods 9 fresh adult cadavers (9 pairs of clavicle specimens) acquired from the civil affairs department, hospital, orphanage, body donation and other ways from January 2009 to March 2015 were randomly divided into the reconstruction plate group and the locking plate group, with 9 clavicle specimens in each group. The biological characteristics of the two groups were compared by the axial compression experiment, three-point bending test and torsion experiment. Results At all levels of load point, the strain and displacement values in the locking plate group were less than those in the reconstruction plate group, the differences were statistically significant (P < 0.05), and the difference increased with the load significantly. In the 600 N of loading, the axial stiffness value, stress shielding rate in the locking plate group were significantly better than those in the reconstruction plate group (P < 0.05). At all levels of the load point, the deflection values in the locking plate group were less than those in the reconstruction plate group, the differences were statistically significant (P < 0.05), and the difference increased with the load significantly. Under the load of 20 N·m, the load bending stiffness of the plate side and opposite side in the locking plate group were significantly greater than those in the reconstruction plate group (P < 0.05). Under the action of all levels of torque, the torsion angle values in the locking plate group were less than those in the reconstruction plate group, the differences were statistically significant (P < 0.05), and the difference increased with the torque significantly. Under the action of 5 N·m torque, the torsional stiffness in the locking plate group was significantly larger than that in the reconstruction plate group (P < 0.05). Conclusion The biomechanical properties of locking plate is better than reconstruction plate for patients with clavicle fracture fixation. (窦庆寅 仰明莉)
