基于品管圈降低给药时患者身份识别方法不正确率的研究(1)
[摘要] 目的 探讨品管圈对降低给药时患者身份识别方法不正确率的效果。 方法 2014年11月~2015年6月在南阳市第一人民医院开展以“降低给药时患者身份识别方法不正确率”为主题的品管圈活动,通过现况调查,运用柏拉图和鱼骨图分析给药时患者身份识别方法不正确的原因,制订实施相应对策,比较品管圈活动前后护士给药时和输液、皮下及皮内注射、发放口服药、更换液体等不同途径给药过程中患者身份识别方法的不正确率,评价活动改善效果。 结果 改善后护士给药时患者身份识别方法不正确率由改善前的37.76%下降至20.59%,差异有统计学意义(χ2=35.847,P < 0.05),改善后输液、皮下及皮内注射、发放口服药、更换液体过程中患者身份识别方法不正确率由改善前的11.18%、8.86%、7.71%和4.43%分别下降到7.25%、5.49%、4.52%和1.57%,差异有统计学意义(P < 0.05),品管圈活动目标达成率为104.6%。 结论 品管圈活动可以降低给药时患者身份识别方法的不正确率,保证护理安全,培养护理人员创新精神,提高工作协调能力、解决问题能力和品管工具应用能力。

[Abstract] Objective To explore the effect of quality control circle on reducing the incorrect rate of methods for patients identification when drug administration. Methods The activities of quality control circle with theme of "reducing the incorrect rate of methods for patients identification when drug administration"were carried out in the First People's Hospital of Nanyang from November 2014 to June 2015, including the cross-sectional investigation, analysis of the reasons for incorrect methods for patients identification by fishbone diagram and platos diagram. The corresponding countermeasures were formulated and implemented. By comparing the incorrect rate of methods for patients identification before and after development of quality control circle when drug administration and in different route of drug administration such as infusion, subcutaneous and intradermal injection, oral medicine and replacing liquid, the effect of quality control circle was evaluated. Results The incorrect rate of methods for patients identification when drug administration was declined from 37.76% to 20.59%, the difference was statistically significant between before and after improvement (χ2=35.847, P < 0.05) The incorrect rate of methods for patients identification in different route of drug administration such as infusion, subcutaneous and intradermal injection, oral medicine and replacing liquid was declined from 11.18%, 8.86%, 7.71% and 4.43% to 7.25%, 5.49%, 4.52% and 1.57% respectively. The difference was also statistically significant (P < 0.05). The target achievement rate of quality control circle was 104.6%. Conclusion The activities of quality control circle can reduce the incorrect rate of methods for patients identification when drug administration and ensure the nursing safety. Meanwhile, it can cultivate the innovative spirit and improve the ability of coordination, problem-solving and quality management tools application in nursing staff. (刘先娟 郭清兰 郭晓娟)
