综合护理干预在断指再植术患者中的应用效果(1)
[摘要] 目的 探究综合护理干预在断指再植术患者中的应用效果。 方法 选取2015年7月～2017年11月合肥市第二人民医院显微骨科收治的62例接受断指再植术患者，按随机数字表法分为对照组与观察组，每组各31例。对照组患者采取常规护理措施，观察组患者在常规护理措施基础上接受血管危象护理、综合心理护理、精细作业康复训练及中医护理干预。两组患者出院时分别统计断指成活率，血管危象发生情况与护理满意度，并在护理干预前与术后3个月通过手指总主动活动度评价法(TAM)、普渡钉板测试(PPT)及明尼苏达手灵巧度评定法(MMDT)评估患指功能恢复情况。 结果 观察组断指再植成活率高于对照组，血管危象发生率低于对照组，术后3个月TAM、PPT与MMDT结果优于对照组，护理满意率高于对照组，差异有统计学意义(P < 0.05)。 结论 优质综合护理干预有效促进断指再植术后患指成活与功能恢复，具有较高的应用价值。

[Abstract] Objective To investigate the application effect of comprehensive nursing intervention on replantation of amputated finger. Methods From July 2015 and December 2017， in Department of Microscopic Orthopaedic， the Second People′s Hospital of Hefei City， 62 patients with replantation of amputated finger were selected. According to random number table method， they were divided into observation group and control group， with 31 cases in each group. The control group was given routine nursing care， the observation group was given vascular crisis nursing， comprehensive psychological nursing， delicate occupational rehabilitation training and traditional Chinese medicine nursing intervention based on routine nursing care. The survival rate and vascular crisis occurrence rate of severed finger， nursing service satisfaction in two groups were compared after patients leaving hospital， before nursing intervention and 3 months after surgery， severed finger function was assessed by total active movement (TAM)， Purdue pegboard test (PPT) and Minnesota manual dexterity test (MMDT). Results The survival rate of the replantation severed finger in the observation group was higher than control group， the incidence of vascular crisis was lower than control group， the results of TAM， PPT， and MMDT were better than control group 3 months after surgery， the rate of nursing satisfaction was higher than control group， the differences were statistically significant (P < 0.05). Conclusion The comprehensive nursing intervention can effectively improve severed finger survival and function rehabilitation after replantation， it has high application value.

近年來，随着显微骨科技术的迅速发展与断指再植术的不断完善，术后断指的存活率显著提高[1]。但因为血管危象等情况的发生，术后断指坏死仍时有出现[2]。由于我国劳动人口众多，此类患者总量较多，进一步提高断指存活率对于改善患者的生产生活意义重大，而术后合理有效的护理干预对于提高手术成功率具有重要作用。此外，断指功能的最大限度恢复更依赖于优质的康复护理[3]。因此，笔者提出了在常规的护理基础上，通过开展包括综合心理护理干预、中医护理干预以及精细作业康复训练等措施的综合护理干预，以进一步提高断指存活率与患指功能。, 百拇医药(许华 营敏荣 樊立云)
