鳖甲煎丸对慢性乙型肝炎早期肝硬化患者肿瘤坏死因子α和基质金属蛋白酶2的影响(1)
摘要目的：觀察鳖甲煎丸治疗慢性乙型肝炎早期肝硬化的临床疗效及其对肿瘤坏死因子(Tumor Necrosis Factorα，TNFα)和基质金属蛋白酶2(Matrix Metalloproteinase2，MMP2)的影响。方法：将80例慢性乙型肝炎早期肝硬化患者纳入研究，随机分为对照组及观察组，各40例。2组患者均接受多烯磷脂酰胆碱胶囊改善肝功能、恩替卡韦抗病毒治疗，观察组在对照组治疗方案基础上加用鳖甲煎丸，2组均接受为期12周的治疗疗程，治疗结束后比较2组患者肝功能、肝纤维化指标、肝脏彩超结果以及外周血清TNFα和MMP2浓度变化。结果：1)治疗后2组患者肝功能、肝纤维化指标、肝脏彩超结果均较治疗前有所改善，其中观察组改善幅度较对照组明显，差异均有统计学意义(P<005)；(2)治疗后2组患者外周血清TNFα和MMP2水平均较治疗前下降，其中观察组下降趋势较对照组明显，差异均有统计学意义(P<005)。结论：鳖甲煎丸可明显改善慢性乙型肝炎早期肝硬化患者病情，其作用机制可能与下调外周血清TNFα和MMP2水平有关。

Influence of Turtle Carapace Decoction Pill on Chronic Hepatitis B Patients with Earlier Period Cirrhosis on TNFα and MMP2

AbstractObjective：To observe the clinical curative of Turtle Carapace Decoction Pill on chronic hepatitis B patients with earlier period cirrhosis，and on TNFα and MMP2.Methods：A total of 80 chronic hepatitis B patients with earlier period cirrhosis from November 2013 to December 2015 in our hospital were studied，and they were randomly divided into the control group (40 cases) and the study group (40 cases).Both of the two groups received polyene phosphatidylcholine capsule to improve liver function，and entecavir antiviral treatment.The study group was treated with Turtle Carapace Decoction Pill based on the treatment of the control group，and then two groups of patients after 12 weeks′ treatment regarding liver function，liver fibrosis indexes，color doppler ultrasound of liver，and the concentration changes of peripheral blood T TNFα and MMP2 were compared.Results：1) The liver function，liver fibrosis indexes and color doppler ultrasound results of both two groups were better after the treatment，and the study group received better results compared with the control group，the differences were statistically significant (P<005)； 2) The TNFα and MMP2 levels in peripheral blood decreased in the two groups after treatment，the downtrend of study group was obvious than that of the control group，the differences were statistically significant (P<005).Conclusion：Turtle Carapace Decoction Pill can obviously improve the health condition of early cirrhosis of chronic hepatitis B patients，and the mechanism may be related to decreased TNFα and MMP2 levels in peripheral blood.
