养阴清肺汤治疗老年慢性支气管炎的临床效果及对患者免疫功能的影响研究(1)
摘要 目的：探讨养阴清肺汤治疗老年慢性支气管炎的临床效果及对患者免疫功能的影响。方法：选取2015年3月至2017年3月周口市中医院收治的老年慢性支气管炎患者166例，按随机数字表法分组，对照组83例予以常规对癥治疗，观察组83例在对照组基础上予以养阴清肺汤治疗，比较2组间血气分析指标水平、血清细胞因子水平、外周血T淋巴细胞亚群水平、临床疗效及不良反应发生率。结果：对照组有效率(81.92%)低于观察组(92.78%)，差异有统计学意义(P<0.05)；与对照组比较，观察组治疗后二氧化碳分压(PaCO2)水平较低，氧分压(PaO2)、动脉血氧含量(CaO2)水平较高，治疗后血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平较低，治疗后外周血CD3+、CD4+T淋巴细胞、CD4+/CD8+比值水平较高，CD8+T淋巴细胞水平较低，差异有统计学意义(P<0.05)；治疗中出现的不良反应为头晕、恶心呕吐、腹泻，2组不良反应发生率差异无统计学意义(P>0.05)。结论：养阴清肺汤治疗老年慢性支气管炎的临床效果较好，能有效改善患者血气分析指标，降低炎性细胞因子水平，提高细胞免疫功能，而且安全性较高。

Abstract Objective：To investigate the curative effects of Yangyin Qingfei Decoction in the treatment of elderly patients with chronic bronchitis and the effects on immune function.Methods：A total of 166 elderly patients with chronic bronchitis in our hospital were selected and randomly divided into 2 groups with 83 cases in each group according to random number table.The control group was treated with routine symptomatic treatment，and the experiment group was treated with Yangyin Qingfei Decoction on the basis of control group.The level of blood gas analysis indexes，serum cytokine levels，the levels of T lymphocyte subsets in peripheral blood，clinical efficacy and incidence of adverse reaction were compared.Results：The effective rate in the control group (81.92%) was lower than that in the experiment group (97.28%)，and the difference was statistically significant (P<0.05)； Compared with the control group，the level of carbon dioxide partial pressure (PaCO2) was lower，and the oxygen partial pressure (PaO2) and the arterial oxygen content (CaO2) were higher after treatment in the experiment group.The levels of serum tumor necrosis factor α (TNF-α)，interleukin-6 (IL-6) and interleukin-8 (IL-8) were lower after treatment.The levels of peripheral blood CD3+，CD4+T lymphocytes，CD4+/CD8+ were higher，and the level of CD8+T lymphocytes was lower after treatment，with significant difference (P<0.05)；the adverse reactions included dizziness，nausea，vomiting and diarrhea，and there was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion：The curative effects of Yangyin Qingfei Decoction in elderly patients with chronic bronchitis is good.It can effectively improve the blood gas analysis index，reduce the level of inflammatory cytokines，and improve cellular immune function with high safety.
