参芪地黄散治疗急性肾损伤患者临床疗效观察及对患者肾功能的影响(1)
摘要目的：探讨参芪地黄散治疗急性肾损伤患者临床疗效及对患者肾功能的影响。方法：选取2015年10月至2016年10月潍坊市中医院收治的急性肾损伤82例，按照随机数字表法分为观察组41例与对照组41例。对照组采用常规西医治疗，观察组在对照组基础上结合参芪地黄散治疗。2组疗程均为8周。比较2组治疗疗效，治疗前后肾功能指标、胱抑素C(CysC)、尿白蛋白排泄率(UAER)变化，及不良反应发生情况。结果：观察组总有效率(90.24%)高于对照组(68.29%)(P<0.05)；2组治疗后血清Scr和BUN水平降低(P<0.05)；观察组治疗后血清Scr和BUN水平低于对照组(P<0.05)；2组治疗后血清Cys C水平降低(P<0.05)；观察组治疗后血清Cys C水平低于对照组(P<0.05)；2组治疗后UAER降低(P<0.05)；观察组治疗后UAER低于对照组(P<0.05)；2组均未发生严重不良反应。结论：参芪地黄散治疗急性肾损伤患者临床疗效明显，且可改善患者肾功能。

AbstractObjective：To investigate the clinical effects of Shenqi Dihuang powder on acute renal injury and its effects on renal function.Methods：A total of 82 cases of acute renal injury admitted in Weifang Hospital of Traditional Chinese Medicine from October 2015 to October 2016 were divided into observation group of 41 cases and control group of 41 cases according to the random number table method.The control group was treated with conventional western medicine， while the observation group was treated with Shenqi Dihuang powder on the basis of the control group.The two groups were treated for 8 weeks.The therapeutic effects of the two groups were compared， and the changes of Cys C， UAER and adverse reactions were observed before and after treatment.Results：The total efficiency of the observation group (90.24%) was higher than the control group (68.29%)(P<0.05)； Serum Scr and BUN decreased after treatment in 2 groups (P<0.05)； After treatment， serum Scr and BUN were lower in the observation group than the control group (P<0.05)； the serum Cys C decreased after treatment in 2 groups (P<0.05)； After treatment， serum Cys C level in observation group was lower than the control group (P<0.05)； the content of UAER decreased in 2 groups after treatment (P<0.05)； After treatment， UAER in observation group was lower than the control group (P<0.05)； No serious adverse reactions occurred in 2 groups.Conclusion：The clinical efficacy of Shenqi Dihuang powder in the treatment of acute kidney injury is significant， and it can improve the renal function of patients.

急性肾损伤主要常见的一种临床危重症，主要是指3个月内功能的结构或者肾脏异常[1]。急性肾损伤临床表现主要为肾功能急剧下降，短时间内血清尿素氮(BUN)、血肌酐(Scr)出现进行生上升，水电解质和酸碱平衡失衡、尿量减少及全身各系统症状等，严重影响患者生命质量。目前，临床一般采用支持性治疗，病死率较高，预后较差[2-4]。近年来研究表明中医药应用于急性肾损伤患者取得良好临床疗效，且临床用药安全性良好[5-6]。本研究旨在探讨参芪地黄散治疗急性肾损伤恢复患者临床疗效及对患者肾功能的影响。
