胫骨近端开放性骨折的治疗选择(1)

【摘要】 目的 探讨胫骨近端开放性骨折的治疗方法以提高其疗效。方法 23例开放性骨折按Gustilo分类，Ⅰ型4例，Ⅱ型9，ⅢA型6例，ⅢB型3例，ⅢC型1例；胫骨近端骨折按AO/ASIF分型，A2型2例，A3型8例，B1型3例，B3型3例，C1型5例，C3型2例；分别行解剖钢板内固定13例和外固定器固定10例。结果 随访时间6～24个月，平均13个月。依Merchant评分标准，内固定组优8例，良3例，可1例，差1例。外固定组优4例，良2例，可2例，差2例。Ⅰ期伤口闭合感染率低于Ⅱ期。结论 对胫骨近端开放性骨折的病人，应视其全身情况及局部软组织条件选择治疗方法。手术复位、牢固固定，Ⅰ期无张力闭合伤口疗效最佳。
