女性膀胱颈部梗阻与糖尿病的关系初步探讨
【摘要】 目的 对49例膀胱颈部梗阻住院患者的症状与糖尿病之间的关系和治疗结果进行研究，同时对二者之间的关系进行初步探讨。方法 1995~2006年共有49例患者因膀胱颈部梗阻住院治疗，其中合并糖尿病者21例(占42.9%)，全部行尿流动力学检查和膀胱镜检查确诊，全部患者给予α1受体阻滞剂治疗；对于药物治疗欠佳者，进行尿道扩张治疗(15例)和经尿道膀胱颈后唇切除术(17例)。结果 大部分患者经药物治疗或尿道扩张治疗后效果良好(32例)，合并糖尿病者药物治疗效果明显差于单纯膀胱颈梗阻患者，其中17例经尿道膀胱颈后唇切除术后，5例仍有排尿功能障碍。结论中老年女性糖尿病患者更易出现排尿功能障碍，对女性膀胱颈部梗阻患者，都应进行血糖尿糖检测，根据结果制定相应治疗方案。【关键词】膀胱颈部梗阻；糖尿病；女性
Bladder neck obstruction in women with diabetes：a pilot study

【Abstract】 Objective To study the relationship between bladder neck obstruction and diabetes in women.Methods From 1995-2006,total of 49 cases suffered from bladder neck obstruction were treated,21 of the 49(42.9%)were diabetes.All the cases were diagnosed occording to urodynamic examination or cystoscope.Inhibitor of receptor-α1 had been used in each cases.Urethral sounding were performed in 15 cases,while resection of hinder margin on bladder neck were performed in the other 17 cases.ResultsSymptoms for bladder outlet obstruction had been ameliorated significantly by drug therapy or urethral sounding in cases without diabetes.For cases with diabetes,however,5 cases were not able to urinate on their own even after surgical procedure.Diabetes may play a role in pathological hyperplasia and hypertrophy indetrusor muscle due to Detrusor Weakness.Urinary tract infection,which caused by diabetes,may accelerate this process.Conclusion Dysuria are common in aged female.Routine test of blood sugar and urine sugar would be instructive in treatment of bladder neck obstruction in women. ......
