丹红注射液对急性脑梗死患者脑循环动力学影响的临床研究(1)
【摘要】 目的 为观察和研究丹红注射液治疗急性脑梗死患者的疗效及对脑循环动力学的影响。方法 对86例该病患者进行脑循环动力学观察，采用随机、双盲和平行对照的方法，把患者分为两组，同样接受脑梗死的基本治疗的基础上，治疗组用生理盐水250 ml加入丹红注射液20 ml，静脉滴注， 1次/d；对照组用生理盐水250 ml加入复方丹参注射液20 ml，静脉滴注，1次/d，均以14 d为1个疗程。对患者进行丹红注射液治疗前、治疗后的脑神经功能缺损评分，同时测定脑循环动力学参数(CVDI)。结果 ①丹红注射液组神经功能缺失，治疗组有效率为90.7%，对照组为79.1%；②丹红注射液组患者病灶一侧CVDI中，脑血流量(Q)及血流速度(V)用丹红注射液治疗后较治疗前显著增加(P<0.05)，较对照组亦有显著增加(P<0.05)。血管阻力(R)、弹性及临界压力(CP)经丹红注射液治疗后，较治疗前明显降低(P<0.05)，较病灶对侧大脑差异亦有显著性差异(P<0.05)。结论 丹红注射液能明显改善脑血流灌注，降低脑循环阻力，减少神经功能缺损，从而达到治疗脑梗死的目的，且安全有效。

【Abstract】 Objective To observe and study the clinical efficacy of danhong injection and its effect on cerebral hemodynamics in patients with acute cerebral infarction ，we choose 86 patients in our hospital in research of cerebral hemodynamics.Methods This clinical trial was randomly double-blind and parallel controlled.The patients were divided into two groups.All patients recieved same basic treatment；The treatment goup was trented by physiological saline(250 ml) plus danhong injection(20 ml) with mainline one time each day.The contrast goup was trented by physiological saline(250 ml) plus fufangdanshen injection(20 ml) with mainline one time each day.The period of trentment is 14 days.then scoring the condition of neural functional deficits before and after the treatment；at the same time the cerebrovascular dynamics index(CVDI) of the patients were measured.Results ①The neural functional deficits of patients in trial group tended to be repaired.The effective rate (ER) was 90.7%，but the control group’s ER was 79.1%.②The total cerebral blood flow and velocity(V)in the focal side of brain after the treatment in the treatment group showed a statistically significant increase，compared with those before the treatment (P<0.05)，and even with the control group(P<0.05).And the cerebral vascular resistance(R)，elasticity and critical pressure (CP) showned a statistically significant decrease (P<0.05).Conclusion These results revealed that the danhong injectionis effective in relieving the neural functional deficits of patients with acute cerebral infarction , reduce cerebral circulation’s resistance.It may attain the goal for treating the cerebral infarction，regarding as a safe and effective drug of the cerebral infarction.

目前丹红注射液已经广泛应用于临床治疗缺血性脑血管疾病，各方面研究的报道较多，但尚未见丹红注射液对急性脑梗死患者的脑循环动力学影响的有关报道。本研究仅就丹红注射液对颈内动脉系统动脉硬化性脑梗死患者神经功能缺失的治疗效果和对脑循环动力学的影响进行了初步临床探讨，现将结果报告如下。
1 资料与方法
1.1 临床资料 2007年1月至2008年1月入住本院的未经治疗的不符合溶栓条件的急性脑梗死患者(发病72 h以内)86例。并随机分为丹红注射液治疗组(简称治疗组)与常规治疗对照组(简称对照组)。其中治疗组43例，男22例，女21例；年龄40~79岁，平均56.2岁；脑动脉硬化22例，合并高血压32例，血脂异常17例，糖尿病18例。对照组43例，男21例，女22例，年龄40~75岁，平均54.5岁，脑动脉硬化11例，合并高血压18例，血脂异常9例，糖尿病12例。两组年龄、性别及病情比较，差异无统计学意义，具有可比性。, 百拇医药(张建斌 张丽玲 王素芳 吴丽丽)
