简易可调式尿管牵引架在膀胱癌根治术后尿管牵引中的效果研究(1)

【摘要】 目的 探讨尿管牵引架在全膀胱切除-原位回肠新膀胱手术后尿管牵引的效果。方法 对2004年1月至2009年9月,我科行全膀胱切除-原位回肠新膀胱手术后使用尿管牵引的80例患者进行分析,术前按手术顺序随机分为实验组40例和对照组40例。对照组采用传统常规尿管皮牵引,实验组采用自制简易可调式尿管牵引架。评价尿管牵引效果,记录术后伤口引流管和尿管停留时间;吻合口愈合情况;是否发生尿漏并发症等。结果 与对照组比较,实验组术后伤口引流管和尿管停留时间短,吻合口愈合快,尿漏并发症发生率低。结论 应用尿管牵引架效果较好,适用于全膀胱切除-原位回肠新膀胱术后患者的辅助治疗。

【Abstract】 Objective To evaluate the efficacy of home-made foley catheter-distractor after operation of radical cystectomy with othotopic ileal neobladder.Methods Retrospectively analysised 80 patients underwent radical cystectomy with orthotopic ileal neobladder in our hospital from Januray 2004 to September 2009, who were applied foley catheter traction postoperatively. The 80 patients were randomly divided the 80 patients into control group and experimental group. The 40 patients in control group were applied routine foley catheter-skin traction. The 40 patients in experimental group were applied foley catheter traction with adjustable foley catheter-distractor. Evaluated the efficacy of foley catheter traction, and collected the data of the retaining time of the drainage tube and foley catheter, the status of theanastamotic stoma, and the incidence of urine-leakage.Results Compared with the control group, the experimental group had relatively shorter retaining time of drainage tube and foley catheter, quicker healing of theanastamotic stoma and lower incidence of urine-leakage.Conclusion Using the home-made foley catheter-distractor could achieve better outcome than routine skin-traction, it could be applied as an adjuvant therapy after radical cystectomy with orthotopic ileal neobladder.

尿管牵引是泌尿外科手术后常用的一种辅助治疗方法, 尤其适用于须行全膀胱切除-原位回肠新膀胱术患者,因术后新膀胱与尿道吻合,通过尿管牵引,能减少吻合口张力,加速吻合口愈合[1]。到目前为止还没有专门的导尿管牵引装置, 传统的尿管皮牵引具有操作简便的特点,但固定尿管在患者大腿外侧的透气胶布易因放置时间长而脱落,以及当患者移动身体时,容易过度牵拉或牵拉力不足,导致吻合口愈合不佳的缺点 ......
