【摘要】 目的 探讨认知行为治疗对精神分裂症患者治疗依从性的影响。方法 对88例精神分裂症患者随机分为研究组44例和对照组44例,研究组在常规药物治疗的同时,辅以认知行为治疗,对照组按常规药物治疗。观察12周,以阴性与阳性症状量表(PANSS)评定病情,计算复发率;以自制依从性调查问卷评价干预效果。结果 研究组复发率为0,干预8周PANSS评分与入组时比较差异显著(P<0.05),干预12周PANSS评分两组比较差异显著(P<0.05);对照组复发率为11.4%(5/44)。12周末两组依从性评分差异具显著性(P<0.05)。结论 认知行为治疗可以提高患者的治疗依从性,减少疾病的复发。

【Abstract】 Objective To explore the influence of cognitive behavior therapy (CBT) on treatment compliance of schizophrenic patients. Methods 88 patients were randomly assigned to study group (n=44) and control group (n=44).Study group were received CBT in addition to conventional medication, while control group were treated with only conventional medication for 12 weeks. The positive and negative symptom scale (PANSS) was used to assess the patient’s condition, and to calculate the recurrence rate. Intervention effect was assessed with self-made compliance questionnaire. Results The recurrence rate of study group was 0. After 8 weeks’ intervention, the scores of PANSS was significant difference than that of the baseline (P<0.05). There was significant difference on the scores of PANSS between the two groups after 12 weeks’ intervention (P<0.05). The recurrence rate of control group was 11.4% (5/44).There was significant difference on the scores of compliance between the two group at 12 weekend (P<0.05). Conclusion Our findings strongly suggest that CBT can improve the patients’ treatment compliance, reduce the disease’s recurrence.

精神分裂症患者,往往不承认自己有病,无自知力,不愿意住院治疗,大多数患者在发病期既影响工作,又降低了生活质量,而一旦症状缓解则又掉以轻心,不按时服药,导致疾病复发。有报道认为认知行为治疗可提高精神分裂症患者治疗依从性[1],为此,我们对88例精神分裂症患者进行了为期12周的认知行为治疗,现将结果报告如下。
1 资料与方法
1.1 一般资料 来自2008年12月至2009年3月在我院住院的精神分裂症患者。入组标准:①符合中国精神疾病分类与诊断标准第3版(CCMD-3)中精神分裂症诊断标准[2];②阴性症状与阳性症状量表评分≤35分。入组共88例,性别:男39例,女49例;年龄:19~45岁,平均(24.5±3.2)岁;婚姻:未婚41例,已婚47例;文化程度:初中及以下48例,高中22例,大专及以上18例;病程:2月~4年,平均(5 ......
