经尿道前列腺切除术后3种膀胱冲洗方法的效果对比研究(1)

【摘要】 目的 探讨3种不同膀胱冲洗方法对经尿道前列腺切除术后保持尿管引流通畅的效果观察。方法 收集2003年10月至2009年10月在我科行经尿道前列腺切除术患者90例,随机分为A、B、C 3组各30例。A组使用间歇冲洗法;B组使用持续冲洗法:C组使用持续冲洗+手动冲洗法。3组膀胱冲洗液均使用等渗生理盐水。患者术后均需留置22号FOLEY硅胶三腔导尿管。对3组尿管引流液澄清程度、血块堵塞尿管、膀胱痉挛等方面的比较。结果 3组患者在尿管引流液澄清程度、尿管堵塞、膀胱痉挛等方面的比较,差异有统计学意义(P<0.05);C组保持尿管引流通畅效果明显,无血块堵塞尿管,亦无膀胱痉挛发生。结论 单一使用持续或间歇膀胱冲洗方法易引起血块堵塞尿管及发生膀胱痉挛,而持续冲洗+手动冲洗方法联合应用,能有效解除血块引起的尿管堵塞,利于保持尿管引流通畅。

Comparison on efficacy of catheter patency maintenance with three methods of bladder irrigation after transurethral resection of prostate

【Abstract】 Objective To compare the draining efficacy of Foley catheter with ttree methods of bladder irrigation after transurethral resection of prostate (TURP).Methods From October 2003 to October 2009,90 patients underwent TURP in our department were randomly assigned into 3 groups,each with 30 patients.All the patients were catheterized with 22F silicon triple way Foley catheters and underwent bladder irrigations with normal saline,with intermittent template in Group A,continuous template in Group B and continuous manualcombination template respectively.Compared the three groups with tubidity of the urine,the incidence of clot obstruction and bladder spasm.Results The difference of tubidity of urine,the incidence of catheter obstruction and bladder spasm between the three groups were statistically significant (P<0.05).Group C have better tubidity of urine,lower incidence of catheter obstruction and bladder spasm.Conclusion Bladder irrigation with continuous or intermittent template could relatively increase the incidence of catheter obstruction and bladder spasm.Continuous manual combination bladder irrigation had better efficacy of catheter patency maintainance by reducing the incidence of catheter obstruction with clots.
