【摘要】 目的探讨亚临床期圆锥角膜的Pentacam角膜地形图特点,为圆锥角膜的早期筛查提供参考。方法 依照Rabinowitz诊断标准,选取亚临床期圆锥角膜组18例20眼,临床期圆锥角膜组26例33眼,正常对照组25例30眼。使用Pentacam测量各组角膜前后表面最大高度、角膜前后表面最适球面屈光度及角膜最薄厚度,统计分析各指标的组间差异。结果 临床期、亚临床期圆锥角膜组和正常对照组的角膜平均后表面最大高度分别为(99±36)μm;(34±17)μm;(15±5)μm。平均前表面最大高度分别为(46±20)μm;(15±8)μm;(8±4)μm。平均后表面最适球面屈光度分别为(56.7±4.6)D;(53.1±1.6)D;(51.2±1.8)D。平均前表面最适球面屈光度分别为(46.2±2.6)D;(43.3±1.2)D;(42.1±1.4)D,平均角膜最薄厚度分别为(432±40)μm;(490±19)μm;(540±25)μm。以上参数在各组之间的差异均有统计学意义(P< 0.01)。亚临床期圆锥角膜组后表面最大高度、后表面最适球面屈光度、前表面最大高度、前表面最适球面屈光度的ROC曲线下面积分别为0.908,0.785,0.765,0.736。 结论 Pentacam角膜地形图检测是圆锥角膜早期诊断的高度敏感性分析的可靠方法,其中后表面最大高度的改变在圆锥角膜筛查过程中尤为重要。

【Abstract】 Objective
To investigate the characteristics of subclinical keratoconus measured by Pentacam. Methods 20 eyes from 18 subclinical keratoconus patients, 33 eyes from 26 keratoconus patients(Rabinowitz diagnosis standards)and 30 eyes from 25 healthy subjects were enrolled in this research. All of them were examined with Pentacam measurement, the posterior and anterior surface maximum elevation, the refractive power of posterior and anterior surface BFS, the corneal thickness was recorded. The correlation of several parameters were statistically analyzed. Results In the three groups(keratoconus, subclinical keratoconus and normal corneas), the maximum elevation of posterior surface was(99±36)μm,(34±17)μm,(15±5)μm. The maximum elevation of anterior surface was(46±20)μm,(15±8)μm,(8±4)μm. The refractive power of posterior surface BFS was(56.7±4.6)D,(53.1±1.6)D,(51.2±1.8)D. The refractive power of anterior surface BFS was (46.2±2.6)D,(43.3±1.2)D,(42.1D±1.4)D. The thickness of corneal thinnest point was (432±40)μm;(490±19)μm;(540±25)μm. There was a statistically significant difference in all the parameters among three groups (P<0.01). In subclinical keratoconus group, the areas under the ROC curves of the posterior surface maximum elevation, the refractive power of posterior surface BFS, the anterior surface maximum elevation, the refractive power of anterior surface BFS was 0 ......
