【摘要】 目的 探讨乌司他丁对创伤性急性肺损伤患者治疗中的疗效。方法 选择60例创伤性急性肺损伤患者,随机分为乌司他丁治疗组(n=30)和对照组(n=30),乌司他丁治疗组在常规治疗基础上静脉滴注乌司他丁20万U,1次/12 h,连续应用5 d,观察两组的临床疗效,具体观察治疗前后呼吸频率(RR)、氧和指数(PaO2/FiO2)、氧分压(PO2)、二氧化碳分压(PCO2)和C-反应蛋白水平变化。结果 乌司他丁治疗组与对照组在治疗前各项指标无差异。治疗后乌司他丁治疗组的C-反应蛋白水平较对照组下降(P<0.05),乌司他丁治疗组后呼吸频率(RR)、氧和指数(PaO2/FiO2)、氧分压(PO2)、二氧化碳分压(PCO2)与对照组差异有统计学意义(P<0.05)。结论 乌司他丁可抑制机体炎症介质的产生,急性肺损伤早期应用治疗效果明显,对急性肺损伤预后有所改善。

The clinical observation of Ulinastatin in the acute lung injury

【Abstract】 Objective Toobserve the effect of Ulinastatin acute lung injury.Methods Select 60 patients with traumatic acute lung injury patients were randomly divided into treatment group Ulinastatin (n=30) and the control group (n=30), Ulinastatin treatment group in conventional treatment on the basis of intravenous drip Ulinastatin200000U, every 12 hours once, continuous application 5 days, observed the clinical curative effect of two groups, specific observed before and after treatment ventilatory frequency (RR), oxygen and index (PaO2/ FiO2), oxygen partial pressure (PO2), carbon dioxide points pressure (PCO2) and C-reactive protein level changes. Results Ulinastatin treatment group and control group before therapy each index compared with control group no difference. After the treatment of Ulinastatin treatment group C-reactive protein level was dropped the control group (P<0.05), had Ulinastatin treatment group (RR) after breathing, oxygen and index (PaO2/ FiO2), oxygen partial pressure (PO2), carbon dioxide points and PCO2 pressure difference was statistically significant (P<0.05). Conclusion Ulinastatin1 can suppress the inflammation of generation, acute lung injury early treatment has obvious effect, application of acute lung injury prognosis is improved.
