【摘要】 目的 观察穿通支原体(Mycoplasma penetrans,Mpe)感染的IgA肾病患者外周血中T细胞亚群的变化,探讨Mpe感染与IgA肾病发病之间的关系。方法 选取2009年1月至2010年6月期间在温州医学院第一附属医院肾内科经肾活检确诊、年龄>18岁的原发性IgA肾病患者118例为病例组,以PCR技术检测外周血中Mpe p35和Genus表达,进一步分为Mpe感染组和非Mpe感染组,以86例同期行健康体检者为正常对照组。以流式细胞术检测外周血中T细胞亚群水平。结果 IgA肾病组患者外周血CD3+、CD4+和CD8+T细胞水平较对照组高,差异有统计学意义(P<0.05),CD4+/CD8+比值较对照组减低,差异有统计学意义(P<0.05)。与非Mpe感染组相比,Mpe感染组患者外周血CD3+、CD4+细胞及CD4+/CD8+比值均减低,差异有统计学意义(P<0.05),两组间CD8+细胞比例比较差异无统计学意义(P>0.05)。结论 IgA肾病患者外周血中T细胞亚群比例失衡,表现为CD3+、CD4+和CD8+T细胞比例升高,CD4+/CD8+比值减低;Mpe感染后,进一步影响T细胞亚群。

The effect of mycoplasma penetrams infection on T cell subsets in peripheral blood of adult patients with primary IgA nephropathy

【Abstract】 Objective To detect the changes of T cell subsets in peripheral blood of adult patients with primary IgA nephropathy infected by Mycoplasma penetrans(Mpe) infection.Methods The expression of Mpe p35 and Genus was detected by PCR in the peripheral blood 118 patients with primary IgA nephropathy were divided into Mpe group and nonMpe group according to the expressions of Mpe p35 and Genus. T cell subsets in the peripheral blood were detected by flow cytometry.Results The levels of CD3+ cells, CD4+ cells and CD8+ cells in the IgA nephropathy group were significantly higher than in control group, while the ratio of CD4+/CD8+ cells were lower in IgA nephropathy group than control group(P<0.05). The levels of CD3+ cells,CD4+ cells and the ratio of CD4+/CD8+ cells decreased significantly in Mpe group than NonMpe group(P<0.05).The levels of CD8+ had no signifiance different between TWO GROUP (P>0.05).Conclusion The levels of T cell subsets disorder in IgA nephropathy patients. Mpe infection could influent the disorder of T cell subsets.
