实时三维超声心动图技术对妊娠期高血压疾病左心功能的研究(2)

准确测量左房容积是有效应用其容积变化评价左心功能的前提,RT3 dE通过三维容积测量,可获得真实立体的左房瞬时容积。研究结果表明,在松弛性下降组,虽心肌松驰性减低,致左室主动充盈减少,因左房代偿性收缩增强,以维持左房与左室间压差,使得左室维持较高的心排量,此阶段虽然左房内压升高,但仍处于代偿阶段。随着PIH病程进展,左室舒张功能的进一步减退,发展至充盈假性正常化及限制性充盈异常时,左室松驰性和顺应性的双重受损,前负荷和后负荷均增加,左房长期做功增加而失去代偿能力,导致LAEF明显降低。
RT3 dE从全新角度直观显示左房形态变化,对左房容积和功能进行定量评价,可以间接判断左室舒张功能受损的程度[8],尤其可用来很好的进行假性充盈正常的识别,指导临床对患者采取有效的保护及治疗措施,减少不良后果的发生。
