【摘要】目的通过分析ST段抬高的急性心肌梗死(STEMI)、非ST段抬高型心肌梗死(NSTEMI)和不稳定型心绞痛(UAP)患者冠脉介入治疗前后血浆中脑钠肽(BNP)水平的变化,探讨血浆BNP水平与急性冠状动脉综合征(ACS)不同类型的关系，并探讨BNP水平能否作为冠脉介入治疗的疗效指标。方法选择经冠状动脉造影证实为ACS的患者120例,按其类型分为3组，其中STEMI组66例,NSTEMI组30例,UAP组24例,对照组30例。对120例ACS患者行经皮冠状动脉介入治疗(PCI)，测定手术前后血浆BNP水平。对照组测定冠状动脉造影前后血浆BNP水平。结果STEMI组及NSTEMI组BNP水平明显高于其他2组(P<005),其中STEMI组高于NSTEMI组(P<005), UAP组高于正常对照组(P<005)，120例ACS患者在经皮冠脉介入术后36 h BNP均明显下降，正常对照组经皮冠脉介入术前后无明显变化。结论不同临床类型ACS患者的血浆BNP浓度存在差异,其水平高低与ACS严重程度相关；ACS患者冠脉介入治疗后血浆BNP含量明显降低,可作为疗效评价指标。

【Abstract】ObjectiveBy analyzing plasma BNP levels of STEMI, NSTEMI and UAP patients before and after coronary artery interventional therapy, to investigate the relationship between plasma BNP and the levels of different types ACS and weather the level of plasma UAP can be adopted as an index of evaluating clinical effect in the treatment of acute coronary syndrome Methods120 cases of confirmed by coronary angiography in patients with ACS, where STEMI in 67 cases, NSTEMI in 30 cases, UAP in 24 cases, 30 cases whose coronary angiography and negative were absorbed as control group The 120 cases of ACS patients were all given PCI, and the plasma BNP levels of all ACS patients were determinated before and after coronary artery interventional therapy， while that of the control group were detected before and after coronary arteriongraphy ResultsIn STEMI group and NSTEMI group，BNP levels were significantly higher than the other two groups (P<005),and STEMI group higher than that in NSTEMI group (P<005), UAP group was higher than that in normal control group (P<005), 120 cases of ACS patients in percutaneous coronary intervention after 36 hours of BNP decreased significantly, while BNP of normal control group showed no significant difference before and after coronary artery interventional therapy ConclusionACS patients of different clinical types of plasma BNP concentration differences, the severity of ACS is associated with the level of plasma BNP ......
