【摘要】 目的 探讨2型糖尿病(T2DM)患者血尿酸 (BUA)与下肢血管病变的关系，探讨其在2型糖尿病下肢血管病变中的作用。方法 82例2型糖尿病患者，根据踝肱指数(ABI)分为外周动脉疾病组(PAD)组和非外周动脉疾病组(NPAD)，同期选择重庆南川宏仁医院健康体检35例作为正常对照(NC)组，采用常规生化学方法测定BUA水平，并分析BUA与ABI、WHR、血糖血压、HOMA-IR等相关代谢指标的关系。结果 PAD组、NPAD组BUA水平均高于NC组(P<0.01);PAD组BUA水平显著高于NPAD组 (P<0.01)。2型糖尿病患者ABI与WHR、BUA、FBG和HOMA-IR呈负相关(r=0.220、0.369、0.281、0.347， P<0.01或 <0.05)。HOMA-IR和BUA为影响T2DM患者ABI的独立相关因素。结论 BUA与T2DM患者ABI有密切相关，可能在T2DM下肢血管病变的发生发展中起着重要作用。

【Abstract】 Objective To investigate the relationship of blood uric acid (BUA) with lower limb vasculopathy in type 2 diabetic patients and explore on roles of BUA in lower limb vasculopathy of type 2 diabetes mellitus. Methods 82 patients with type 2 diabetes mellitus were divided into peripheral arterial disease (PAD) group and non-peripheral arterial disease (NPAD) group according to the ankle brachial index (ABI), and meanwhile choose 35 patients of health examination as control groups (NC group). BUA levels were measured by Conventional methods of biochemistry. The relationship between BUA levels, WHR, blood glucose and press and HOMA-IR, other metabolic parameters was also analyzed. Results BUA levels in patients with PAD group and NPAD group were significantly increased compared with NC group (P<0.01); BUA levels were higher in PAD group than that NPAD group (P<0.01). ABI was correlated negatively with WHR, BUA, FBG and HOMA-IR (r=0.220、0.369、0.281、0.347， P<0.01 or <0.05).HOMA-IR and BUA were independent related factors in influencing ABI in T2DM patients. Conclusion BUA levels were well correlated with ABI in T2DM patients. BUA levels may play an important role in the occurrence and development of lower limb vasculopathy in T2DM.
