【摘要】 目的 探讨导乐分娩联合硬膜外镇痛的效果和价值。方法 选择60例待产妇随机分为A组(对照组，n=30，传统分娩+硬膜外镇痛)和B组(观察组,n=30，导乐分娩+硬膜外镇痛)。观察记录两组产程时间、疼痛的强度、分娩方式、阴道出血、胎儿宫内窘迫发生率、催产素使用率及新生儿Apgar评分。结果 B组产程比A组缩短，镇痛效果优于A组(P<0.05)；B组缩宫素使用率、剖宫产率和产后出血量均少于A组，差异有统计学意义(P<0.05)，两组胎儿宫内窘迫发生率及新生儿Apgar评分比较，差异无统计学意义(P>0.05)。结论 导乐分娩联合硬膜外麻醉镇痛效果更好，分娩中采取有效的全程陪伴护理措施，减轻产妇的紧张、焦虑情绪，明显缩短产程, 减少剖宫产率和产后出血量。

Efficacy evaluation of Doula childbirth with epidural analgesia

【Abstract】 Objective
To investigate the application effects of Doula childbirth with epidural analgesia in delivery rooms. Methods Sixty cases with singlefetus mature pregnant women were randomised into two groups with 30 cases each. Group A (control group) received traditional delivery + epidural analgesia and group B (observation group) received Doula delivery + epidural analgesia. The stages of labor, pain intensity, delivery pattern, vaginal bleeding, incidence rate of fetal intrauterine distress, use rate of Oxytocin and the Apgar score of fetus delivered were recorded during each labor women. Results The stages of labor in group B was significantly shorter than that in group A(P<0.05).Analgesia effect was considered significantly better in group B than in group A(P<0.05).Use rate of Oxytocin, the rate of cesarean section and vaginal bleeding after delivery in group B were significantly less than those in group A(P<0.05).The incidence rate of fet al intrauterine distress and Apgar scores differences in the two groups were not significant (P>0.05). Conclusion Analgesia effects of Doula childbirth with epidural analgesia is much better. Taking effective nursing measure with whole process accompany during delivery can reduce pregnant women’s nervous emotions and anxiety, shortens fetus delivery time, and decrease the rate of cesarean section and vaginal bleeding after delivery.
