晚期胃癌合并幽门梗阻的治疗分析
【摘要】目的探讨晚期胃癌合并幽门梗阻的治疗。 方法回顾性分析25例晚期胃癌合并幽门梗阻患者的手术原则及化疗方法。 结果围术期死亡2例，其中胃肠吻合术1例，胃造瘘术1例。其余胃肠吻合术者11例术后生存2.6~8.1个月；姑息性肿瘤切除术者9例，术后平均生存1.3年；胃癌根治术(联合脏器切除)者3例，生存2.2年。腹腔灌注化疗较全身静脉化疗副反应轻，效果好。 结论晚期胃癌合并幽门梗阻诊断、治疗复杂。晚期胃癌合并幽门梗阻宜早期手术，合理治疗，以减轻患者痛苦改善全身状况和提高生存质量为主。手术辅以化疗可以延长患者生存期，提高生活质量。【关键词】晚期胃癌；幽门梗阻；手术；化疗

【Abstract】ObjectiveTo investigate the treatment of late gastric carcinoma with pyloric obstruction. MethodsThe treatment principles and chemotherapy on 25 cases of late gastric carcinoma with pyloric obstruction were retrospectively analyzed. Results2 cases died during operation including, one with gastroenteroanastomosis and the other one with gastrostomosis. The average survival time was 2.6~8.1 months of the other 11 cases with gastroenteroanastomosis. 9 cases received palliative subtotal gastrectomy with the average survival time being 1.3 years. 3 cases received radical gastrectomy with survival time being 2.2 years. The peritioneal cavity perfusion might improve side reactions and significant effect. ConclusionWe ought to do rational therapy which might delay patiens life, and operate as quickly as possible. It is important to lighten the suffering to improve the condition of all over the body and to elevate quality of life for advanced carcinoma of stomach. ......
