【摘要】目的探讨血液透析滤过治疗尿毒症顽固性高血压的临床疗效。方法本文对收治的尿毒症顽固性高血压患者采用血液透析滤过治疗，并与进行血液透析治疗的HD组进行临床效果比较。结果HDF组显效16例，有效14例，总有效率达96.77%；HD组显效10例，有效13例，总有效率达74.19%；经统计学分析发现HDF组的显效率和总有效率明显高于HD组，且差异具有统计学意义(P<0.05)。治疗4周后，HD组和HDF组患者收缩压、舒张压及血浆RA、AngⅡ浓度均明显下降，与治疗前比较差异有统计学意义(P<0.05)，HDF组治疗后的上述各项指标与HD组治疗后比较下降更显著(P<0.05)。结论血液透析滤过可以显著降低尿毒症顽固性高血压患者的收缩压和舒张压，可使血浆RA、AngⅡ浓度明显下降，从而使顽固性高血压得到控制，且减少心脑血管并发症的发生，值得临床推广应用。

【Abstract】ObjectiveTo investigate the clinical efficacy of hemodiafiltration in treating uremic refractory hypertension. MethodsIn this paper, uremic refractory hypertension patients were treated with hemodiafiltration and comparing the clinical efficacy with the HD group patients who were treated with hemodialysis. ResultsHDF group of 16 cases markedly effective in 14 cases, total effective rate was 96.77%.HD group of 10 cases markedly effective, 13 cases, the total effective rate was 74.19%.The statistical analysis showed that the efficiency HDF group and total effective rate was significantly higher than HD group, and the difference was statistically significant (P<0.05). After 4 weeks of treatment, HD patients group and the HDF systolic pressure, diastolic blood pressure and plasma RA, Ang Ⅱ levels are significantly decreased compared with before treatment there was significant difference (P<0.05), HDF group after treatment with the above indicators HD group decreased more significantly after treatment (P<0.05). ConclusionThe hemodiafiltration can significantly reduce blood pressure in uremic patients with refractory systolic and diastolic blood pressure, can plasma RA, Ang Ⅱ levels were significantly decreased, so that resistant hypertension under control, and reduce cardiovascular and cerebrovascular complications occurred, worthy of clinical application.
