手术治疗联合光动力疗法治疗皮肤恶性肿瘤的临床效果(1)
【摘要】 目的 分析手术治疗联合光动力疗法治疗皮肤恶性肿瘤的临床效果。方法 44例皮肤恶性肿瘤患者(包括26例基底细胞癌患者、8例鲍温病患者、6例鳞癌患者、4例湿疹样癌患者)作为研究对象， 均采用手术治疗联合光动力疗法进行治疗， 分析治疗后的临床效果、病灶的复发率以及出现的不良反应情况。结果 44例患者经过手术切除治疗后， 有35例患者完全切除了病灶， 其余9例患者进行了部分病灶切除；在术后进行3次局部光动力学治疗后， 患者的皮损症状完全消失。术后6个月内无一例患者出现疾病复发症状；在治疗1年后， 共有5例患者出现了疾病复发， 复发率为11.4%。术后手术部位出现轻度的疼痛感和灼烧感及轻微的红肿疼痛， 2 d内不适症状自行消退；部分患者出现显著的肿痛症状， 给予冷敷护理。结论 光动力疗法是皮肤恶性肿瘤手术治疗的重要辅助疗法， 能够有效的降低手术切除部位的深度及面积， 对于复杂手术具有重要意义， 然而降低联合治疗方法时疾病的复发率， 还需要继续探索。

Clinical effect of surgery combined photodynamic therapy in the treatment of skin malignant tumor WANG Jun. Skin Surgery Outpatient Service， Shenyang City Seventh People’s Hospital， Shenyang 110003， China
【Abstract】 Objective To analyze clinical effect of surgery combined photodynamic therapy in the treatment of skin malignant tumor. Methods A total of 44 skin malignant tumor patients(including 26 cases of basaloma， 8 cases of Bowen’s disease， 6 cases of squamous carcinoma and 4 cases of eczematoid carcinoma) as research subjects all received photodynamic therapy. Analysis was made on clinical effect， recurrence rate of lesions， and incidence of adverse reactions after treatment. Results There were 35 patients had complete removal lesions after operation removal in 44 patients， and 9 patients had partial removal lesions. Skin damage completely disapeared after 3 times of local photodynamic therapy. No patients had symptoms of recurrence within 6 months postoperation. 5 patients had recurrence after 1-year treatment， with recurrence rate as 11.4%. Mild pain， burning sensation， slight redness and swelling pain showed on surgery spot after operation， then disappeared within 2 days. Partial patients with clear redness and swelling pain received cold compress nursing. Conclusion Photodynamic therapy proves to be an important adjuvant therapy for skin malignant tumor， which can decrease depth and area of surgical removal effectively， and it has great significance for complicated operation. Exploration should be made on decreasing the recurrence rate with combination therapy method.
