青白联合手术治疗合并白内障的闭角型青光眼疗效分析(1)
【摘要】 目的 分析采用青光眼白内障联合手术(青白联合手术)治疗合并白内障的闭角型青光眼的临床疗效。方法 47例合并有白内障的闭角型青光眼患者， 对所有患者均实施超声乳化白内障摘除人工晶体植入联合小梁切除术治疗， 分析治疗效果。结果 患者治疗前眼压为(40.53±6.75)mm Hg (1 mm Hg=0.133 kPa)， 治疗后眼压为(12.52±2.86)mm Hg， 患者治疗后眼压明显低于治疗前， 差异有统计学意义(t=26.1942， P=0.0000<0.05)。患者治疗前视力为(0.43±0.14)， 治疗后视力为(0.82±0.21)， 患者治疗后视力明显高于治疗前， 差异有统计学意义(t=10.5936， P=0.0000<0.05)。患者治疗前眼房角开放度为(86.69±18.93)°， 治疗后眼房角开放度为(64.23±16.69)°， 患者治疗后眼房角开放度明显低于治疗前， 差异有统计学意义(t=6.1013， P=0.0000<0.05)。所有患者均未出现任何并发症和不良反应。结论 采用青白联合手术治疗合并白内障的闭角型青光眼， 疗效显著且安全， 有较高的应用价值。

【Abstract】 Objective To analyze clinical effect of angle-closure glaucoma combined with cataract treated by trabeculectomy with cataract surgery. Methods A total of 47 cases of angle-closure glaucoma combined with cataract patients were treated by phacoemulsification and intraocular lens implantation combined with trabeculectomy. Clinical effect was analyzed. Results Patients had intraocular pressure before treatment as (40.53±6.75) mm Hg (1 mm Hg=0.133 kPa) and (12.52±2.86) mm Hg after treatment， there was a lower intraocular pressure after treatment than before treatment， and difference had statistical significance (t=26.1942， P=0.0000<0.05). Patients had eyesight as (0.43±0.14) before treatment and (0.82±0.21) after treatment， there was a higher eyesight after treatment than before treatment， and difference had statistical significance (t=10.5936， P=0.0000<0.05). Patients had eye corner openness before treatment as (86.69±18.93)°and (64.23±16.69)°after treatment， there was a lower eye corner openness after treatment than before treatment， and difference had statistical significance (t=6.1013， P=0.0000<0.05). No patients showed complications and adverse reaction. Conclusion Impelement of trabeculectomy with cataract surgery for the treatment of angle-closure glaucoma combined with cataract proves significant clinical effect， high security and application value.

白内障以及青光眼均为老年人群中的常见病以及多发病， 同时也会对患者的身体健康和生活质量造成较大影响。因此， 对合并白内障的闭角型青光眼患者实施安全有效的治疗非常重要。但在对合并白内障的闭角型青光眼患者进行治疗时， 较常见的单纯超声乳化手术以及超声乳化联合房角分离术并不能够取得较好的治疗效果[1]。超声乳化白内障摘除人工晶体植入联合小梁切除术开始得到了较多关注。本次研究即分析了对合并白内障的闭角型青光眼患者实施青白联合手术的治疗效果， 现报告如下。, 百拇医药(冉颖)
