彩超引导下细针穿刺诊断甲状腺结节的应用价值(1)
【摘要】 目的 研究彩超引導下细针穿刺活检(US-FNAB)在甲状腺结节诊断中的临床价值。方法 在彩超引导下对41例甲状腺结节(直径0.5~1.5 cm)患者行US-FNAB, 分析检查结果, 与术后病理进行对比, 从而判断US-FNAB的准确率。结果 患者手术后病理结果:甲状腺良性结节7例(结节性甲状腺肿 3例, 桥本氏甲状腺炎1例, 腺瘤3例), 其中US-FNAB 诊断良性病变 5例, 无法判断病理类型1例, 取材过少无法诊断1例。甲状腺癌 34例(乳头状癌 32例, 髓样癌 1例, 滤泡性癌1例), 其中US-FNAB诊断恶性结节32例, 无法判断病理类型1例, 取材过少无法诊断1例。US-FNAB诊断甲状腺结节准确性、敏感性分别为90.2%和95.1%。US-FNAB细胞学结果与手术病理的一致性Kappa=0.694, χ2检验P=0.480>0.05, 提示 US-FNAB与手术病理结果诊断的一致性良好。41例行 US-FNAB穿刺的患者均未发生出血、声音嘶哑、吞咽和呼吸困难等明显并发症。结论 US-FNAB应用于甲状腺结节的诊断准确率高, 且具有操作快速、安全的特点, 是诊断甲状腺结节安全有效的方法。

【Abstract】 Objective To research clinical value of color Doppler ultrasound-guided fine needle aspiration biopsy (US-FNAB) in diagnosis of thyroid nodule. Methods US-FNAB was applied in 41 thyroid nodule cases (diameter 0.5~1.5 cm) under guidance by color Doppler ultrasound, and test outcomes were analyzed. Comparison was made on the outcomes and postoperative pathological outcome to evaluate accuracy of US-FNAB. Results Postoperative pathological outcome in patients showed 7 cases with benign thyroid nodule (3 cases with nodular goiter, 1 case with Hashimoto's thyroiditis and 3 cases with adenoma), including 5 benign cases diagnosed by US-FNAB, 1 case with undeterminable pathological pattern and 1 undiagnosed case due to insufficient sampling. There were 34 cases with thyroid cancer (32 cases with papillary carcinoma, 1 case with medullary carcinoma and 1 case with follicular carcinoma), including 32 cases with malignant nodule diagnosed by US-FNAB, 1 case with undeterminable pathological pattern and 1 undiagnosed case due to insufficient sampling. In diagnosis of thyroid nodule, US-FNAB showed accuracy and sensitivity respectively as 90.2% and 95.1%. US-FNAB cytological outcome and surgical pathological outcome had consistency Kappa as 0.694 and P in χ2 test as 0.480>0.05, and US-FNAB and surgical pathological outcome had good diagnostic consistency. There were no obvious complications of hemorrhage, hoarseness, dysphagia and dyspnea in 41 cases. Conclusion Application of US-FNAB shows high accuracy in diagnosis of thyroid nodule. Containing advantages of quick operation and safety, it is a safe and effective method in diagnosis of thyroid nodule., 百拇医药(王凤云 孙怀玉 张慧娟)
