上颌前牙区接受单颗种植修复患者应用不翻瓣微创种植的效果评价(1)
【摘要】 目的 评价上颌前牙区接受单颗种植修复患者应用不翻瓣微创种植的效果。方法 60例上颌前牙区接受单颗种植修复患者, 按照随机数字法分为对照组和观察组, 各30例。对照组患者给予翻瓣种植, 观察组患者给予不翻瓣种植。在种植体愈合6个月以后进行冠修复。在手术后6、12个月时对患者进行锥形束CT影像学检查, 同时对种植体唇舌侧骨量变化情况进行评价。在完成冠修复6、12个月对种植体牙周探诊深度、龈乳头指数、改良齦沟出血指数进行检测, 并使用红色美学评分对种植体进行美学评价。结果 观察组在6、12个月时唇侧骨吸收量和舌侧骨吸收量均少于对照组, 差异有统计学意义(P<0.05)。观察组患者改良龈沟出血指数在冠修复即刻时高于对照组, 差异有统计学意义(P<0.05)。观察组患者龈乳头指数在完成冠修复6个月时高于对照组, 差异有统计学意义(P<0.05)。观察组在6、12个月时红色美学评分分别为(9.60±1.96)、(10.10±2.05)分, 均高于对照组的(8.20±1.56)、(9.10±1.65)分, 差异有统计学意义(P<0.05)。结论 在上颌前牙区接受单颗种植修复患者中应用不翻瓣种植手术, 既可促进周牙龈乳头的成形, 又能使唇舌侧骨吸收量减少, 具有临床美学效果, 可广泛推广于临床。

【Abstract】 Objective To evaluate the effect of minimally invasive flapless implant for patients with maxillary anterior teeth undergoing single implant repair. Methods A total of 60 patients with maxillary anterior teeth undergoing single implant repair were divided by random number method into control group and observation group, with 30 cases in each group. The control group received flap implant, and the observation group received flapless implant. The crown was repaired 6 months after implant healing. Patients were examined by cone beam CT imaging in postoperative 6 and 12 months, and the changes of labial and lingual bone mass were evaluated. In 6 and 12 months after completion of crown restoration, detection were made on periodontal exploration depth, gingival papilla index and improved gingival sulcus bleeding index, and the implant was aesthetically evaluated by red aesthetics score. Results The observation group had less labial and lingual bone mass in 6 and 12 months than the control group, and the difference had statistical significance (P<0.05). The observation group had higher improved gingival sulcus bleeding index at the moment of crown repair than the control group, and the difference had statistical significance (P<0.05). The observation group had higher gingival papilla index in 6 months after crown repair than the control group, and the difference had statistical significance (P<0.05). The observation group had red aesthetics score in 6 and 12 months respectively as (9.60±1.96) and (10.10±2.05) points, which were higher than (8.20±1.56) and (9.10±1.65) points in the control group. Their difference had statistical significance (P<0.05). Conclusion For patients with maxillary anterior teeth undergoing single implant repair, application of flapless implant surgery can not only promote the forming of gingival nipple, but also can decrease the labial and lingual bone absorption, and it has clinical aesthetic effect.
