跨学科护理团队在全髋关节置换患者快速康复中的作用(1)
【摘要】 目的 分析在全髋关节置换患者快速康复中应用跨学科护理团队的效果。方法 72例全髋关节置换患者， 采用随机信封法分为对照组和干预组， 每组36例。对照组患者采用常规护理， 干预组患者在常规护理基础上联合跨学科护理团队护理。观察比较两组患者髋关节恢复情况及住院时间、治疗费用、护理满意度、并发症发生情况。结果 干预组患者的疼痛评分、功能评分、畸形评分、活动度评分均显著高于对照组， 住院时间及治疗费用少于对照组， 差异均具有统计学意义(P<0.05)。干预组患者的护理满意度97.2%(35/36)显著高于对照组的80.6%(29/36)， 差异具有统计学意义(χ2=5.063， P<0.05)。干预组患者并发症发生率2.8%(1/36)显著低于对照组的16.7%(6/36)， 差异具有统计学意义(χ2=3.956， P<0.05)。结论 将跨学科护理团队应用于全髋关节置换患者快速康复中， 康复效果显著， 既可缓解临床指标、减少并发症发生几率， 又可提高髋关节恢复水平及患者满意度， 值得临床推广应用。

【Abstract】 Objective To analyze the effect of interdisciplinary nursing team in the rapid rehabilitation of patients with total hip arthroplasty. Methods A total of 72 patients with total hip arthroplasty were divided by random envelope method into control group and intervention group， with 36 cases in each group. The control group received conventional nursing， and the intervention group received interdisciplinary nursing team nursing on the basis of conventional nursing. Observation and comparison were made on hip recovery， hospitalization time， treatment costs， nursing satisfaction， and occurrence of complications between the two groups. Results The intervention group had significantly higher pain score， functional score， deformity score， activity score than the control group， shorter hospitalization time than the control group， and less treatment cost than the control group. Their difference was statistically significant (P<0.05). The intervention group had significantly higher nursing satisfaction as 97.2%(35/36) than 80.6%(29/36) in the control group， and the difference was statistically significant (χ2=5.063， P<0.05). The intervention had significantly lower incidence of complications as 2.8%(1/36) than 16.7%(6/36) in the control group， and the difference was statistically significant (χ2=3.956， P<0.05). Conclusion Application of interdisciplinary nursing team in the rapid rehabilitation of patients with total hip arthroplasty has a remarkable effect. It can not only alleviate clinical indicators， reduce the incidence of complications， but also improve the recovery level of hip joint and patient satisfaction. It is worthy of clinical promotion and application.

目前， 全髖关节置换临床较为常见。其中全髋关节是由人工股骨头、人工髋臼组成。其多应用于老年股骨颈骨折患者中， 主要是由于老年人机体功能下降、骨质疏松等因素的影响， 当其受刺激后， 易降低骨骼的稳定性， 从而增加骨折发生几率[1]。在全髋关节置换治疗过程中， 由于不同的影响， 易对全髋关节置换术造成影响[2]。而成功的全髋关节置换术包括良好的心理、病理学、麻醉、护理、康复、营养等， 故实施科学的跨学科护理团队， 对提高全髋关节置换效果至关重要[3]。本次研究取2014年1月～2017年2月收治的72例全髋关节置换患者进行研究， 分析在全髋关节置换患者快速康复中应用跨学科护理团队的效果， 现报告如下。, http://www.100md.com(王国秀 刘永霞)
